Anyone who will be entering the building who doesn’t have swipe access must fill out this waiver. Please check in at the front desk upon entry with your first and last name, saying you filled out a Smart Waiver for the Questrom Cohort Cup event. Please check in this way every time you enter/exit the building, and you only need to fill out the waiver once.
