Bonclarken Waiver for Carmel Baptist Youth Retreat
Before participating in this event, you must complete and sign the required document below.
Participants under the age of 18:
You will need to have a parent or guardian complete the Youth Retreat (Ages 0-17) | Bonclarken Conference Center Waiver and Release from Liability on your behalf.
Parents/guardians: You will only need to fill out one waiver for all participating minors from your household! After selecting Minor, you will indicate the number of participating minors and enter their name and birthdate information using the slots provided.
Participants ages 18 or older:
You will need to complete the Youth Retreat (Ages 18 and Older) | Bonclarken Conference Center Waiver and Release from Liability on your own behalf.
