Rec & PE Family Membership Application: Duke Affiliate's Minor Dependents

Please Read Thoroughly and Sign Below.

  • I agree the terms of this agreement will remain valid as long as the person affiliated with Duke (student, faculty, staff, alumni) also referred as Head of Household or Primary Account Member, remains part of Duke University and continue to remain in good standing with Duke Rec & PE.
  • I understand if the Head of Household voluntarily separates or am terminated by Duke University the membership is no longer valid.
  • I agree and understand the membership is extended to the employee, spouse and/or dependents (ages 10-26) when applicable.
  • If the relationship of the employee and spouse no longer exists, the employee is required to remove said individuals from the plan immediately.  
  • Spouses and Dependents agree to present their valid DukeCard (if they have a NetID) or their MyRec Mobile Digital ID (if they use OneLink) at the front desk of Wilson Recreation Center and Brodie Recreation Center upon each visit.
  • I understand that all of the Rec & PE Memberships are non-transferable.

Risk Waiver and Release

I understand that participation in all activities in the Wilson/Brodie Center (“WBRC”) are voluntary and participants are responsible for their own welfare. I agree to take personal responsibility for using proper footwear and clothing while in the recreation facilities. I understand that Duke University strongly recommends that each participant have personal medical coverage.

While on Duke’s premises, I will adhere to Duke’s policies and regulations including but not limited to parking, traffic and security regulations and with all other ordinances, laws and regulations that may be required by Federal, State and local governments or insurance and health agencies. I further assume responsibility and liability for all damage to property caused, resulting from, or arising out of my use of Duke’s facilities and Duke’s premises. All my activities will be at my own risk, and I am hereby given notice of responsibility to make arrangements to guard against physical, financial, and other risks as appropriate.

I understand that Duke shall have no responsibility for the loss, theft, mysterious disappearance of, or damage to personal property which may be brought into WBRC; except for damage caused by direct and sole negligence of Duke.

I agree to indemnify and hold harmless Duke, including its trustees, officers, directors, employees and agents, from any claim, damage, liability, injury, expense, including reasonable attorney’s fees or loss arising directly or indirectly out of its’ use of Duke facilities and Duke premises.

I certify that the information I have provided is correct and I agree to abide by all rules and regulations governing the recreational facilities. I have read and understand all policies and procedures of WBRC and accept responsibility for abiding by all regulations and policies, which may from time to time be reasonably adopted.

Spouse and Dependent Sponsorship Process

Spouses and Dependents not part of the Duke system (without NetID) must create their own unique Duke OneLink account, and add it to our Rec Grouper Group in order for them to be added to our systems and have access to all Rec & PE facilities and services.

The Duke Affiliate (referred as the Head of Household and primary account Member) is responsible for complying with Duke’s policies as well as applicable laws and regulations, in regard to their Spouse and Dependents. Should a family member violate Duke’s policies or applicable laws,their account and membership may be suspended and/or terminated.

Process for Spouses and Dependents without a Duke Net ID

1. The Spouse and each Dependent need to visit MyRec and SIGN UP to create a Duke OneLink account.

  • If you already have a OneLink account, proceed to the next step.
  • Complete the process until each account is verified.

2. The Spouse and each Dependent need to visit and log in with their verified OneLink account to our Rec Grouper Group (LINK) and confirm you have JOINED the group.

  • Once completed, you will be able to use your OneLink account to sign in MyRec, register for programs and classes, use our free MyRec Mobile app, etc.

3. Download our FREE MyRec Mobile, as you will be required to present and scan the application's Digital ID to gain access to all our facilities.

  • Once completed, you will be able to use your OneLink account to sign in MyRec, register for programs and classes, use our free MyRec Mobile app, etc.
  • Upon your first visit, you will need to present a valid form of ID and allow our staff to add your photo (for future verification) in our systems.


Process for Spouses and Dependents with a Duke Net ID

1. The Spouse and each Dependent need to visit and log in with their Net ID to our Rec Grouper Group (LINK) and confirm you have JOINED the group.

  • You will be able to use your Duke Net ID to sign in MyRec, as well as use your Duke Card or Duke Mobile Card for entry.
