StrikePoint Adult & Minor Participant Waiver – Renninger’s – Effective 10/2/2026

For purposes of this agreement, “Participant” means me if I am participating as an adult and/or each minor child listed on this agreement for whom I am the natural guardian.

1. ACKNOWLEDGMENT OF RISKS AND SAFETY RULES

I understand that mobile laser tag and related inflatable or interactive activities are active indoor and outdoor recreational activities. Inherent and other risks may include running, quick changes of direction, low-light conditions, uneven or wet surfaces, slips, trips, falls, collisions with participants or objects, equipment or projectile contact, physical exertion, overexertion, heat, weather exposure, loud sounds, flashing or strobe-like lights, and reactions to competitive play.

These risks may result in strains, sprains, cuts, bruises, aggravation of a medical condition, serious injury, disability, death, or property damage. I understand that not every risk can be identified, avoided, or eliminated, even when reasonable care is used.

I confirm that each Participant is physically and medically able to participate or that relevant medical conditions, accessibility needs, or activity limitations have been disclosed to StrikePoint staff. Each Participant will follow all posted and verbal safety rules, boundaries, equipment instructions, start and stop commands, and emergency directions.

Each Participant agrees not to engage in intentional physical contact, tackling, pushing, wrestling, reckless running, equipment misuse, or other unsafe conduct. Laser-tag participants must be at least eight years old. StrikePoint may pause play or remove any Participant whose conduct is unsafe or disruptive, without a refund where permitted by law.

2. ADULT PARTICIPANT ASSUMPTION OF RISK AND RELEASE

This section applies if I am at least 18 years old and will participate in the activity.

I voluntarily choose to participate and knowingly assume the inherent and other risks of participation, whether known or unknown.

TO THE FULLEST EXTENT PERMITTED BY FLORIDA LAW, I RELEASE MOUNT DORA ENTERTAINMENT, LLC D/B/A STRIKEPOINT LASER TAG, ITS OWNER, MEMBERS, MANAGERS, EMPLOYEES, AGENTS, VOLUNTEERS, CONTRACTORS, EQUIPMENT PROVIDERS, AFFILIATES, SUCCESSORS, ASSIGNS, AND THE EVENT PROPERTY OWNER (COLLECTIVELY, THE “RELEASED PARTIES”) FROM CLAIMS FOR PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE ARISING FROM THE INHERENT RISKS OF THE ACTIVITY OR THE ORDINARY NEGLIGENCE OF A RELEASED PARTY.

This adult release does not apply to gross negligence, reckless conduct, intentional misconduct, or any duty or right that cannot legally be waived. If any provision is determined to be unenforceable, the remaining provisions will continue in effect.

3. FLORIDA MINOR PARTICIPANT SECTION

This section applies if I am signing for a Participant under 18 years old.

I certify that I am the minor Participant’s natural guardian as recognized by Florida law and that I possess the legal authority to consent to the minor’s participation and execute this agreement. A party host, chaperone, relative, or other adult who is not the minor’s natural guardian should not sign this agreement unless that person possesses the required legal authority.

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF  MOUNT DORA ENTERTAINMENT, LLC D/B/A STRIKEPOINT LASER TAG AND FLORIDA TWIN MARKETS, INC., AS THE EVENT PROPERTY OWNER/HOST, TOGETHER WITH EACH OF THEIR RESPECTIVE OWNERS, AFFILIATES, EMPLOYEES, AND AGENTS USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM  MOUNT DORA ENTERTAINMENT, LLC D/B/A STRIKEPOINT LASER TAG AND FLORIDA TWIN MARKETS, INC., AS THE EVENT PROPERTY OWNER/HOST, TOGETHER WITH EACH OF THEIR RESPECTIVE OWNERS, AFFILIATES, EMPLOYEES, AND AGENTS IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND  MOUNT DORA ENTERTAINMENT, LLC D/B/A STRIKEPOINT LASER TAG AND FLORIDA TWIN MARKETS, INC., AS THE EVENT PROPERTY OWNER/HOST, TOGETHER WITH EACH OF THEIR RESPECTIVE OWNERS, AFFILIATES, EMPLOYEES, AND AGENTS HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.

I have read this entire agreement and the applicable safety rules. I understand the activity’s inherent risks and voluntarily consent to the minor Participant’s participation.

To the extent authorized by Florida Statute §744.301, I waive and release, for myself and on behalf of the minor Participant, all claims against the Released Parties identified in Section 2, expressly including Florida Twin Markets, Inc. as the event property owner/host, for personal injury, including death, or property damage resulting from inherent risks of the activity. This minor release is not intended to waive claims beyond what Florida law permits.

4. EMERGENCY ASSISTANCE

I authorize StrikePoint staff to obtain reasonable first aid or emergency medical assistance for me and/or each listed minor Participant if needed. I understand that StrikePoint does not provide medical insurance and that I am responsible for resulting expenses except as otherwise required by law. This authorization does not require StrikePoint staff to provide care beyond their training.

I will disclose relevant allergies, medical conditions, accessibility needs, or activity limitations before participation.

5. OPTIONAL PHOTO / MEDIA AND MARKETING CHOICES

Separate selectable questions concerning photo and media authorization and optional marketing communications appear below. These choices are separate from the liability waiver. Declining either authorization does not affect eligibility to participate or make a purchase.

6. FINAL CERTIFICATION AND ELECTRONIC SIGNATURE

By signing electronically, I certify that I am at least 18 years old and am signing for myself as an adult Participant and/or as the natural guardian of each listed minor Participant, as applicable.

I have read this entire agreement, understand that it affects legal rights, have had an opportunity to ask questions, and sign voluntarily. I certify that the information I provided is accurate and that I possess the legal authority required for every signature I provide.

I agree that my electronic signature has the same legal effect as a handwritten signature. Florida law governs this agreement. If any provision is unenforceable, the remaining provisions will remain in effect.
