Copy of Certifications
In order to process your recertification, SOLO needs scans or images of the following certification cards. If you do not have copies, please make a note in the field below. If we need to look up your certification, you will be charged an additional $10 lost-card processing fee. READ CAREFULLY - YOU MUST UPLOAD 1 DOCUMENT OR IMAGE OF ALL YOUR CERTIFICATIONS - THE SYSTEM WILL NOT ALLOW FOR MULTIPLE UPLOADS.

Upload an image or file of your current WFR or WEMT card, WFA card (if a SOLO WFA was taken for recert), and an image of your CPR card. SOLO WFR and WEMT certifications are invalid without an active (non-expired) CPR certification.
