Once we receive this application, SOLO will invoice you via PayPal via Retail@soloschools for all associated fees. Receipt of your PayPal invoice indicates your card is being processed. In the event we have questions, someone from our office will email you for any additional information needed to complete your application.

In order to process your recertification, SOLO needs scans or images of the following certification cards. If you do not have copies, please make a note in the field below. If we need to look up your certification, you will be charged an additional $10 lost-card processing fee. READ CAREFULLY - YOU MUST UPLOAD 1 DOCUMENT OR IMAGE OF ALL YOUR CERTIFICATIONS - THE SYSTEM WILL NOT ALLOW FOR MULTIPLE UPLOADS.

Upload an image or file of your current WFR or WEMT card, WFA card (if a SOLO WFA was taken for recert), and an image of your CPR card. SOLO WFR and WEMT certifications are invalid without an active (non-expired) CPR certification.
