Waiver Terms 1. Active Supervision & Child-to-Adult Ratios This is a parent/guardian-supervised facility. Parents and guardians are solely and fully responsible for actively monitoring and supervising their own children at all times. Staff are present to facilitate a clean and orderly environment, not to supervise individual children. A maximum of four (4) children may be accompanied by a single supervising adult. Children under two (2) years of age are designated to play in the separate baby/toddler space for their safety; older children are not permitted in this area. 2. Playground Rules & Conduct Policies To ensure safety, all guests must strictly adhere to the posted facility rules. No shoes are allowed on the jungle jumparoo at any time. All guests must remove soiled shoes upon entering. Inappropriate use of play equipment significantly increases the risk of injury and is strictly prohibited. Outside food and drink are permitted in designated areas, and a coffee station is available for adult guests. Alcohol, nicotine (including vaping/e-cigarettes), and illicit substances are strictly prohibited on the premises. Play equipment must be used safely and respectfully. If a guest or child fails to follow rules, they will receive one warning. Any subsequent violation will result in immediate removal from the premises without a refund. Parents/guardians assume full financial responsibility for replacing any broken equipment or toys if damage occurs when rules were not being followed. 3. Health & Illness Policy In order to maintain a hygienic environment for all families, parents must not bring children to the facility if they have been ill recently or are currently symptomatic (including but not limited to fever, cough, runny nose, vomiting, diarrhea, or contagious rashes). By entering, you certify that all participants in your party are healthy and symptom-free. 4. Equipment Maintenance & Mandatory Incident Reporting We inspect our equipment regularly, but active play can cause sudden wear. If you notice any equipment in disrepair, you must alert facility staff immediately. If you fail to alert staff of a visible defect or issue, you assume all responsibility and liability for any injury that occurs as a result of using that equipment. In the event of any injury or incident on-site, a formal incident report must be filled out and signed by both the parent/guardian and a staff member before leaving the premises. If an incident is not reported to staff prior to your departure, you waive all rights to a subsequent incident report or any related claims. 5. Assumption of Risk & Release of Liability Participation in indoor physical play involves inherent risks, including but not limited to scrapes, bruises, falls, collisions, sprains, or more serious injuries. You acknowledge these risks on behalf of yourself and the minors listed in this waiver. To the fullest extent permitted under Michigan law, you voluntarily assume all risks associated with participation and agree to hold harmless, release, and indemnify the facility, its owners, employees, and agents from any and all liability, claims, actions, or demands arising out of your or your children's use of the facility, except in cases of gross negligence or willful misconduct. Sign & Complete Consolidated Agreement & Execution By signing below, you acknowledge that you have read and understood all sections of this waiver, including the playground rules, the health/illness policies, the equipment reporting mandates, and the assumption of risk/liability release. You confirm you are the parent or legally authorized guardian of the minor children listed on the previous page. I have read, understood, and agree to all the terms, playground rules, and policies set forth in this waiver on behalf of myself and my listed child(ren). I Agree Date: October 25, 2026 |
