You must discontinue use of these medications for a minimum of 3 months prior to waxing. With Accutane, you must discontinue use of this medication for a minimum of 1 year prior to waxing. CAUTION: If you are currently using any of the following, please inform your service provider. These products make the skin more sensitive. Thin, sensitive skin is more vulnerable to lifting and sensitivity during waxing. Other acne medications not listed above

Salicylic Acid PRECAUTIONS & CONSIDERATION: You must wait a minimum of seven (7) days before waxing after a light chemical peel or microdermabrasion. Waxing cannot be performed if you have had any laser skin resurfacing within the past year. Waxing cannot be performed if you have had a physician administered peel within the past two (2) years. Sunburned, irritated areas, cold sores and moles cannot be waxed. Please note that waxing can have certain side effects such as skin removal, redness, swelling, tenderness, etc. I understand and have read and completed the above questionnaire truthfully. I agree that this constitutes full disclosure and thus supersedes any previous verbal or written disclosures. I certify that I have read and fully understand the above paragraphs and that I have asked any questions I have prior to my service. I understand that withholding information or providing misinformation may result in contraindications and/or irritation to the skin from waxing treatments received. I am aware that it is my responsibility to inform the service provider of any current medical or health conditions and to update this history. I have given an accurate account of the questions asked above including all known allergies and/or medications I am currently ingesting or using topically. I am willing to follow recommendation made by my service provider for an at home care regimen that can minimize or eliminate possible negative reactions. In the event I may have additional questions or concerns regarding my treatment or suggested home care product or post-treatment care, I will consult the salon immediately. I give my permission to my service provider to perform the waxing procedure we have discussed. I understand that while my service provider will take every precaution to minimize or eliminate negative reactions that may occur and will hold Indigo Salon and its service provider harmless from any side effect or injury that may result from this treatment. The treatments I receive here are voluntary and I release Indigo Salon and its service provider from liability and assume full responsibility thereof. Salon Policies Acknowledgement I understand that all prices for services are a starting price and additional charges may be added to compensate for added time or use of product to due to length/density of hair, or added time it takes to complete a service. I understand that children and guests are not allowed in the salon or spa unless they are receiving a service. I acknowledge this is due to liability and safety issues and to ensure a relaxing experience for all salon and studio guests. I have read and understand Indigo Salon’s cancelation policy. I am aware that for the salon and studio to run on schedule and to create the best possible experience for all guests, if I arrive more than 10 minutes to my appointment I will be marked as a “no-show”, my entire service will need to be rescheduled and I will be held responsible for paying my cancellation fee in full before booking my next services. I consent to photographs being taken of my services to be used inside Indigo Studios and for the purposes of marketing and social media. I understand that Indigo Salon is owned and operated by a licensed professional and that by law they have the right to refuse service on any client at any time. I verify that I understand and agree to the terms and conditions above for receiving services at Indigo Salon/Studios. POLICIES & FAQ POLICIES Cancellations/No-Shows/Late Arrivals All appointments cancelled or rescheduled within the days 2 weeks (14 calendar days) prior to any scheduled appointment will require a cancellation/rescheduling fee of 100% of the services booked regardless of reason. Any future appointments will not be booked until the fee is paid in full. Any subsequent, pre-booked appointments will be cancelled if payment is not received in full within a week's (7 calendar days) time of notification of cancellation. If you do not show up for your appointment and no notice is given, you will be invoiced and charged full price for all services booked. Cancelling or rescheduling an appointment requiring a retainer will be considered a willful forfeiture of the retainer and a new retainer fee will be required previous to rescheduling.
More than one cancellation, no show, or reschedule without proper notice may result in requiring a non-refundable retainer for all future services booked or in Indigo no longer booking you as a client. If you are more than 10 minutes late for your appointment, regardless of reason, you will be held to the standard of the above cancellation policy and will require the payment of said cancellation fee of 100% of your services booked. Any future appointments will not be booked until the fee is paid in full. Indigo Elite Membership Indigo Elite Memberships are non-transferable, non-refundable and expire 6 months after inactivity. Gift Certificates Purchased gift certificates do not expire. Purchased gift certificates are also non-transferable and non-refundable. Gift certificates acquired by donation have a strict expiration date and typically expire 6 months from the date of the event unless otherwise indicated. Expired gift certificates will not be honored. If an expired certificate is presented, you will be required to pay for the cost of the service. Please check the expiration date prior to booking your appointment. Gift certificates acquired by donation are non-transferable & non-refundable. Frequently Asked Questions Can I get a quote over the phone, via text, via email? The best way to get an accurate quote on price is to book a complimentary consultation. A consultation can be booked as an add on to the service you intend to get or can be booked separately to ensure we have enough time for the services you need. Please feel free to email me for a general quote but a direct quote will not be given via email or over the phone. (This policy has been adjusted due to COVID-19 contact restrictions.) Parking? Where is the best place to park? There is free, 2 hour street parking in front of the building along Court E and the surrounding blocks. Can I bring my child/children with me to my appointment? I do not allow unsupervised children in the salon/studio. If you are getting a service done, I ask for the safety and efficiency of your service that we eliminate as many distractions as possible. If you are unable to secure child care during the time of your service, please feel free to reschedule and also please keep my cancellation policy in mind. Can I bring my dog with me to my appointment? No, however, Service/Therapy dogs are always welcome. Please give proper notice before hand. May I bring a guest with me to my appointment? No. Guest of clients will need to find elsewhere to wait during your appointment time. Can I drop my child off for their appointment and pick them up when they are done? Any person under the age of 18 must be supervised by a parent or legal guardian. |
