Lash Envy Academy (Lash Envy LLC DBA Lé Esthetics)Trainee Waiver

 

This agreement releases Lash Envy LLC DBA Lé Esthetics from all liability relating to injuries that may occur during my visits. By signing this agreement, I agree to hold Lash Envy LLC DBA Lé Esthetics free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

I also acknowledge the risks involved in the care rendered. These include but are not limited to injury, and damage to, or loss of personal property. I declare that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

By signing below I forfeit all right to bring a suit against Lash Envy LLC DBA Lé Esthetics and Lash Envy Academy for any reason. In return, I will receive care. I will also make every effort to obey safety precautions as listed in writing and/or as explained to me verbally by the provider(s). I will ask for clarification when needed.

I have read, agree, and completely understand this consent form.

TRAINEE CONSENT FORM

Lash Envy LLC and the owner and/or staff of Lash Envy LLC accepts no responsibility or liability for any outcome related to these suggested practices or processes during your eyelash extension training for certification in lash extension application or any other type of esthetics training, offered by LASH Envy LLC. This includes but is not limited to lash lift and tint, waxing, facials and eyebrow lamination training. By signing this waiver I agree to all of the statements above
