WAIVER, LIABILITY AND INDEMNITY AGREEMENT

Acknowledgment of Risks:

I, the undersigned, as the parent or legal guardian of the above-named child, acknowledge that participation in activities held by Lil Ninja Academy Pte. Ltd (UEN 202026376C) and Free Solo Ninja Pte Ltd (UEN 202141505H), in this document referred to as (“Lil Ninja Academy”), involves inherent risks and dangers, including but not limited to physical injury. I understand that these risks cannot be completely eliminated, even with the exercise of reasonable care and safety precautions.

Assumption of Risk:

I hereby voluntarily assume all risks associated with my child's participation in the activities, including but not limited to injuries related to the use of gym equipment, falls, and other activities conducted within the facility.

Compliance and Safety:

I understand that the safety of all participants is a priority at Lil Ninja Academy. I acknowledge that the gym staff reserves the right to stop my child from participating in activities if they are not adhering to instructions, engaging in unsafe behaviour, or creating a risk to themselves or others. In such cases, my child may be asked to leave the gym, and no refunds or credits will be issued for any missed sessions.

Release and Waiver:

In consideration of the acceptance of my child’s participation in the activities and programs offered by Lil Ninja Academy, I hereby agree to release, waive, discharge, and hold harmless Lil Ninja Academy, its owners, employees, agents, and representatives from any and all claims, demands, liabilities, costs, and expenses arising out of or in any way connected with my child's participation in the activities.

Media Release:

I acknowledge and agree that Lil Ninja Academy may take photographs and/or videos of my child during activities. I consent to the use of these photographs and/or videos for promotional, marketing, and social media purposes by Lil Ninja Academy. This may include, but is not limited to, posting on Lil Ninja Academy's website, social media platforms, and other marketing materials. I understand that these images and videos will be used solely for the purposes mentioned above and will remain the property of Lil Ninja Academy.

Medical Authorization:

In the event of an emergency, I authorize Lil Ninja Academy and its representatives to seek medical treatment for my child if I cannot be reached immediately. I agree to be responsible for any medical expenses incurred as a result of such treatment.
