Please note that this Liability Waiver should only be signed after you’ve read the MUD Ceramics Studio Handbook and Terms & Conditions.

As a Member of and/or Class Participant at MUD Ceramics Studio, I understand that it is my obligation to adhere to all rules and policies outlined in the MUD Ceramics Studio Handbook and Terms & Conditions. I acknowledge that I may not work at the studio and/or take part in a class/classes unless I have fully familiarized myself with these rules and policies. By my signature and today’s date below, I affirm that I have received, read, and understood the MUD Ceramics Studio Handbook and Terms & Conditions.

I realize that making work and/or taking a class/classes at MUD Ceramics Studio may involve some risk of personal injury, property damage, and even death. Therefore, I voluntarily assume all such risks related to these activities. I hereby release MUD Ceramics Studio, its directors, officers, and employees from any and all liabilities, claims, demands, actions, causes of actions, costs, and expenses of any nature whatsoever, even if arising from the negligence of MUD Ceramics Studio, related to any loss, damage, or injury that may be sustained by me or to any property belonging to me.

By providing the emergency contact details, I authorize MUD Ceramics Studio to contact this individual in the event of an emergency.

I, the undersigned parent or legal guardian of the minor named in this agreement, hereby give my full consent and approval for my child to participate in the ceramics class/membership offered by MUD Ceramics Studio.

I understand that participation in ceramics activities involves inherent risks, including but not limited to, the use of potentially hazardous materials and equipment. I hereby release, hold harmless, and indemnify MUD Ceramics Studio, its employees, agents, and volunteers from any and all liability, claims, and demands arising from my child's participation in these activities.

Medical Treatment Authorization

In the event of an emergency, I authorize MUD Ceramics Studio to seek medical treatment for my child when I or the emergency contact identified above cannot be reached or if a delay would be dangerous for my child's health.

Photography Release

I grant MUD Ceramics Studio permission to take and use photographs and/or digital images of my child for use in promotional materials.
