Welcome to our Spa Membership program!

This program is designed for individuals who want to commit to regular wellness services. Once you sign up, you are entitled to reduced pricing on Massage, Facial and/or Nail services, and can receive as many services per month at that reduced price as you'd like! The membership type and personal elements are all highly flexible and can be changed at any time. In the waiver to follow, you will go through an overview of our options and craft a custom membership to suit your needs! We are so excited to be able to continuously serve you, and look forward to your time with us!

Card on File Policy

We are requiring all of our guests to store a card on file with us to ensure a contact-free payment. Additionally, our 24-hour cancellation policy necessitates having a card on file in the event of a no-show or other policy violation (we charge 50% of service value for a same-day cancellation and 100% of the service value for a no-call no-show). Your card will be stored securely in our encrypted system. Even if you pay for your services using a different payment (cash, check, etc.), you must still provide a card on file.

Your card information will not be stored in this document. If you do not yet have one on file, you are welcome to call it in to (541) 334-6533 or provide it in-store at the time of your next service.

Membership Overview

 Please review the below and always contact us when you have questions. We are here to serve you!


  • There is a $50 non-refundable administration fee to enroll in the Spa Membership program. This is a one time fee to be paid at time of enrollment. If you are filling out this form at home, you can give us a call at (541) 334-6533 and process your Spa Member Enrollment Fee remotely.
  • Services must be received within each calendar month or they are forfeited and your card will be charged the minimum fee of the membership you have chosen. Gervais extends a 5 day grace period to use your membership each month so you have until the 5th of the following month to use your service. 
  • You pay at the time of visit using a card on file.
  • You can opt out of or pause your membership at any time. There are no renewals or cancellation fees. If you do wish to cancel, you must provide written notice 30 days prior to your cancellation date. You can email your cancellation to membership@gervaiseugene.com. 
  • Cancellations for spa service appointments require 24 hours notice to change, reschedule or cancel. Violation of this policy will result in charging the visit to your card on file.
  • Once per month, you can gift your member service to a guest. It will be charged to your account as if it were you receiving the service.
  • Most communication is via email so make sure to keep your email address is always current.
  • You will receive 10% credit back on all retail purchases while you are a Spa Member that can be used to purchase product. The credit will be issued Dec 1st of each year or at the last date of your membership if cancelled. 
  • If Gervais has a price increase for services, you will receive 60 days notice of price increase and can cancel at any time with no penalty or fees.

ESSENTIAL SPA MEMBERSHIP

  • For guests who want to get at least 1 massage and/or facial per month.

  • You can choose unlimited of the following services each month:

Essential Facial (60 minutes for $95 each)

Essential Massage (60 minutes for $95 each)

Restorative Facial (90 minutes for $145 each)

Restorative Massage (90 minutes for $135 each)

  • If you miss a month, you will be charged $95, the minimum service requirement for this membership type.

NAIL SPA MEMBERSHIP 

  • For guests who want to get at least 1 nail service per month.

  • You can choose unlimited of the following services each month:

Spa Manicure (60 minutes for $45 each)

Gel Manicure (75 minutes for $53 each)

Spa Pedicure (75 minutes for $63 each)

Express Manicure & Pedicure (75 minutes for $83) Express Manicure & Pedicure must be booked together for each visit. These services cannot be booked separately.

  • If you miss a month, you will be charged $45, the minimum service requirement for this membership type.

ULTIMATE SPA MEMBERSHIP 

  • For guests who want to get at least 1 massage and/or facial AND at least 1 nail service per month.

  • Includes Essential and Spa Nails memberships with unlimited access to the following services each month:

Essential Facial (60 minutes for $95 each)

Essential Massage (60 minutes for $95 each)

Restorative Facial (90 minutes for $145 each)

Restorative Massage (90 minutes for $135 each)

Spa Manicure (60 minutes for $45 each)

Gel Manicure (75 minutes for $53 each)

Spa Pedicure (75 minutes for $63 each)

Express Manicure & Pedicure (75 minutes for $83) Express Manicure & Pedicure must be booked together for each visit. These services cannot be booked separately.

  • If you miss a month, you will be charged $140, the minimum service requirement for this membership type.

By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it.
