This signed waiver is required for event participation at TEGA Kids. Please include all minors that will be participating. Children under the age of 4 must have an adult present during birthday parties. Adults may observe from our designated viewing areas, but adults are not allowed on the gym equipment at any time. Please dress your child for the appropriate event and active play. We look forward to having you join us! 

*Waivers must be updated every 12 months. 


I understand that participation is entirely by my own choice and with the understanding that there is risk and the possibility of accidental injury in any activity involving unusual motion or height, and that accidental drowning may occur, regardless of skill level, during any activity involving water.

I hereby consent to my child(ren) participating in the programs offered by Texas Elite Gymnastics Academy, Inc (TEGA). I acknowledge that my child(ren) has/have medical approval and is/are physically fit and able to assume the role as a participant. In consideration for allowing my child to use TEGA's equipment and facilities, I hereby forever release Texas Elite Gymnastics Academy, Inc. and 82L, LLC, it's owners, officers, employees, teachers, and coaches from all liability for any and all damage and injuries, negligent or otherwise, suffered by my child while under the instruction, supervision or control of Texas Elite Gymnastics Academy, Inc. and 82L, LLC, it's owners, officers, employees, teachers or coaches. I further understand that while payment of tuition and registration fees constitutes a part of the consideration due to TEGA, an additional and important part of the consideration due to TEGA is this signed release form.

Occasionally TEGA uses photos or video of students in print ads, on its website, commercials, or other marketing mediums. I understand that my child’s likeness may be used in such advertising. These images will not be given or sold to outside companies or individuals.

This acknowledgement of risk and waiver of liability, having been read thoroughly and understood completely, is signed voluntarily as to its content and intent.
