Dissection Lab Session Policies

Parents/Legal guardians must sign off on this form before child(ren) participate in a dissection lab.

Please read the following IMPORTANT Little Beakers' policies regarding lab rules, lab fee refunds and makeups.

 

Classroom

Parents, grandparents, caretakers or guardians are not permitted in the classroom.  Any exceptions will be made at Little Beakers management's sole discretion.
Any student who engages in disruptive or unsafe behavior, as deemed by Little Beakers staff, will be reported to the student's parent, guardian or caregiver.
Little Beakers reserves the right to, and at its sole discretion may, exclude a student from a class and/or any future classes due to disruptive or unsafe behavior.  Lab fees will not be refunded.
For a given lab session / specimen, Little Beakers Management at its sole discretion may have participants work individually or in teams.

Refunds

The lab fee is non-refundable.
A missed dissection lab session may be rescheduled to a future dissection lab session at the Sandy, UT location, subject to availability and Little Beakers management's approval.

 I understand and acknowledge that by electronically signing this Agreement, I am agreeing to be bound by its terms and conditions. MY ELECTRONIC SIGNATURE INDICATES THAT I HAVE READ THE ENTIRE AGREEMENT, UNDERSTAND THAT IT AFFECTS MY LEGAL RIGHTS, AND UNDERSTAND THAT IT WAIVES MY RIGHT TO SUE COMPANY.
