Informed Consent for Procedure

PLEASE READ THE FOLLOWING STATEMENTS CAREFULLY AND INDICATE THAT YOU UNDERSTAND


1. I absolutely understand and accept that this procedure is a process, often requiring multiple Applications of color to achieve desirable results and the 100% success cannot be guaranteed.

2. This agreement, I understand can be a full session followed by ONE additional session. Additional appointments will incur a fee.

3. It has been explained to me and I understand that minor and temporary bleeding, bruising, redness, swelling, fading, or loss of pigment may occur. There is a rare risk of infection, missed place pigment, allergic reaction, fever blisters and/or color change with any cosmetic micro pigments.

4. If I had a permanent cosmetics procedure performed previously by another practitioner, I do not hold TAYLORED BROWS or any of its employees responsible for future allergic reactions or contraindications. 

5. I have informed TAYLORED BROWS and any of its employees of any health problems.

6. I understand that TAYLORED BROWS or any of its employees cannot guarantee the outcome of any Permanent makeup procedure due to the unpredictability of the human skin.

7. I accept responsibility for helping to determine the color, shape, and position of eyebrows.

8. I have received, reviewed and understand the pre and post-procedural instructions as given to me and agree to follow them. I understand the importance of strictly adhering to such instructions. 

9. I understand that any cosmetic surgery can change the positioning of my permanent makeup.

10. If I am a lens wearer, I realize that I should take them out before my appointment.

11. If I insist on driving without my contacts I waive all responsibility to TAYLORED BROWS and any of its employees and I assume full responsibility that I can see to drive, perfectly.

12. I understand that this procedure will fade and this fading can alter the original pigment color due to circumstances beyond the control of TAYLORED BROWS and any of its employees. 

13. I understand that I will need to maintain the color with future applications. Sun, skin care products, pool and other factors play a role in fading as mentioned in the after care instructions.

14. I realize this is an elective cosmetic procedure, not an exact science, and is not medically necessary. No matter the outcome there are no refunds upon treatment for this elective procedure.

15. I authorize TAYLORED BROWS and any of its employees unrestrictive use of before and after photographs/ videos/ reviews to include, but not limited, to portfolio and teaching. TAYLORED BROWS and my technician must have photos of all clients even if they choose not to post them.

16. I give my consent to TAYLORED BROWS and any of its employees to confer with my physicians for medical information required for the safety of my procedures. 

17. I understand that many lasers & IPL’s (Intense Pulse Lights) including those used for hair removal, anti-aging, Photo Facials, removal of lines may or will turn permanent make up color dark or even black. I agree to inform my esthetician or anyone operating such that I have permanent make up. 

18. I am aware that if an infection occurs after I have received Permanent Cosmetics I will see my Primary Care Physician or an emergency room, immediately.

19. I am aware that my practitioner and all of TAYLORED BROWS employees will use new pre-sterilized needle(s), pigment(s), and supplies for all procedures and will follow OSHA standards and on all clients’ new gloves are worn for all procedures.

20. I understand that a patch test does not guarantee that I may not develop and allergic reaction in the future.

21. I understand that my follow up is done during my perfecting touch-up. My practitioner cannot see the true results until a minimum of 5 weeks from my initial appointment (which I have been fully explained and understand this is the full skin healing process). TAYLORED BROWS is not responsible for following up with me post procedure. If I have any concerns it is my responsibility to reach out. I understand that follow-up appointments in between sessions are not necessary and TAYLORED BROWS has the right to charge for additional appointments in between sessions.

22. I understand that TAYLORED BROWS has the right to deny me service or drop me as a client.

23. Your signature below represents consent for Permanent Cosmetic services and shall remain in effect during the entire period you remain a client of TAYLORED BROWS. If anything is to change I will make the staff aware and will resign new paperwork.

24. I acknowledge by signing this consent form. I have been given the full opportunity to ask any and all questions about permanent makeup procedures and processes from my permanent makeup practitioner and/or her associates.

Everything You Need To Know About Your Microblading Appointment

What Is Microblading?

Microblading is a manual “hand” method of permanent cosmetics for your eyebrows which creates extremely fine natural looking strokes by implanting pigment under the epidermis.

What is Blade and Shade

The combo brow is the best of both! Microbladed hair strokes to create texture and detail at the front of the brow (manual hand method) and blended shading (rotary tool) through the body and tail.

How long does Microblading last?

Depending on your skin type in care, the pigment will fade slightly over 18 to 24 months.

Is Microblading or shading painful?

The service can be a little uncomfortable for those who are more sensitive. However, there is an anesthetic applied during to keep this to a minimum to where it is completely tolerable.

How does it work?

The pigment is applied with a specific tool that has a row of several fine needles to create thin hair like marks in the skin. Your artist will choose needles and the blade type according to the technique and the skin type. The pigment is bladed just below the epidermis and the most shallow part of the dermis, making extremely fine natural looking hair strokes. A very natural result is achieved, as the hair strokes are bladed into the skin according to the natural hair growth direction. A touch up is needed 6-8 weeks after your initial treatment.

How long does the service last?

The appointment should take about 2-3 hours.

Are there any pre-existing conditions that interfere with consideration for a Microblading treatment?

• Pregnancy

• Breastfeeding (Doctors note is required)

• On keloids or if you have a tendency to keloid, birthmarks or moles.

• Diabetic.

• Serious diseases such as cancer, epilepsy, autoimmune disorders (doctor's note is required)

• Circulatory disorders (doctor's note is required)

• Any bleeding disorders (doctor's note is required)

• You're taking blood thinning medication

• Currently on Accutane or other strong retinoids (must wait until 6 months after treatment ends, doctors note is required.)

• If any skin diseases symptoms or irritation appears on the area.

• You've recently had Botox or Disport treatment (must wait 2 weeks)

• Old PMU

• You have a broken capillary in the eyebrows area

• If you have a sunburn.

• After waxing

• After chemical peels (must wait 4 weeks)

HOW TO PREPARE FOR YOUR MICROBLADING APPOINTMENT

One week before your appointment avoid alcohol, aspirin and Ibuprofen. It is suggested you stop taking omega 3, B6, vitamin E and fish oil 3 weeks prior to your appointment as they contribute to thinning the blood and may affect how well the numbing works and how well the pigment takes. Do not discontinue any prescribed medication without your doctors permission. You may continue your vitamins 72 hours after your appointment.

On the day of your appointment you’ll want to shower and wash your hair the morning of because you will not be able to shower for 72 hours. After the 72 hours showers should be quick (5 minutes long) for the remainder of your 14 day healing process. Showers should be cool and the door left open to prevent steam.

Wear comfortable clothing. Bringing a sweater can help keep you warm and relaxed.

Avoid waxing, threading, tweezing and electrolysis around your eyebrows for a minimum of 5 days but the longer the better.

Bringing in inspiration pictures of brows you like and dislike will help communicate your aesthetic goals.

Do not tan two weeks prior to the service, or have a sunburned face

Do not have any type of facial 2 weeks prior to the procedure

Do not work out the day of the procedure

Do not have botox 2 weeks prior to the procedure

Do not consume coffee before your procedure

Microblading After Care

The day of the treatment: Absorb

After the procedure, gently blot the area with clean tissue to absorb excess lymph fluid. Do this every 5 minutes for the full day until the oozing has stopped. Removing this fluid prevents hardening of the lymphatic fluids.

Days 1-7: Wash

Wash daily to remove bacteria, build up of product and oils, and dead skin. (Don't worry, THIS DOES NOT REMOVE THE PIGMENT!)

Gently wash your eyebrows each night with water and an antibacterial soap like Dial Soap or Cetaphil. With a very light touch, use your fingertips to gently cleanse the eyebrows. Rub the area in a smooth motion for 10 seconds and rinse with water ensuring that all soap is rinsed away. To dry, gently pat with a clean tissue. DO NOT use any cleansing products containing acids (glycolic, lactic, or AHA), or exfoliants.

Days 1-7 Moisturize

Apply a rice grain amount of aquaphor twice a day with a cotton swab and spread it across the treated area. Be sure not to over-apply, as this will suffocate your skin and delay healing. The ointment should be barely noticeable on the skin. NEVER put the ointment on a wet or damp treated area.

Here are some extra tips to help with a smooth and easy recovery:

Important Reminders

Use a fresh pillowcase while you sleep.

Let any scabbing or dry skin naturally exfoliate away. Picking can cause scarring or loss of color.

No facials, botox, chemical treatments or microdermabrasion for 4 weeks.

Avoid hot, sweaty exercise for 14 days.

Avoid direct sun exposure or tanning for 4 weeks after the procedure. Wear a hat when outdoors.

Avoid long, hot showers for the first 14 days.

Avoid sleeping on your face for the first 14 days.

Avoid face-down swimming, lakes, and hot tubs for the first 14 days,

Avoid topical makeup and sunscreen on the area for the first 14 days.

DO NOT rub, pick, or scratch the treated area.

Remember, with the proper prep and aftercare routine, you will have much better results with your microblading procedure.

Important note about showering:

Limit your showers to 5 minutes so that you do not create too much steam. Keep your face/procedure area out of the water while you wash your body, then, at the end of your shower, wash your hair. Your face should only be getting wet only at the end of the shower. Avoid excessive rinsing and hot water on the treated area.

Your new eyebrows will go through several phases during the healing cycle. True reflection of healed color can only be assessed 3-4 weeks after treatment. It will go from too dark, to too light, to just right!

Immediately after the procedure, the pigment will appear very sharp and dark. This is because the pigment is still sitting on top of your skin and has not yet settled in completely. The color of the pigment will soften gradually (do not be alarmed if you see some pigment on the cotton swab as this is excess pigment and/or body fluid that is naturally exiting your skin).

Once the healing of the skin starts taking place it will look like dandruff flakes or dry skin. This might give you the impression that the color pigment is fading too quickly. However, this is just superficial color and dry skin being naturally removed from your eyebrows.

Your eyebrows will be the lightest tones during days 5-12 because the color is absorbed in the deeper layers of the skin. Later skin cells push it back to the upper layers. Once the healing is completed, you will enjoy a pair of beautiful, new, natural looking eyebrows.

Some residual swelling is normal for ALL procedures. This area may appear uneven, darker, itchy, tender, red and irritated. These symptoms will dissipate each day and vary on an individual basis.

CONSULT A DOCTOR IF THE SKIN ON OR AROUND THE EYEBROWS APPEARS TO BE INFECTED.

Color WILL fade/soften from 30%-60%. At the touch up we will fine tune any area that has faded too much. Healing is specific to each client. It is important to understand that you will need a color boost every year to maintain its fresh natural appearance. Fading will happen after each procedure. We do not have control over our bodies. You may need to powder and/or pencil even after healed results. This is an enhancement to your natural brows. Permanent makeup is an art, not a science. Remember, no two sides of the face are the same. Perfection is our goal but brows are sisters not twins.
