Release and Waiver of Liability
Monkey Rock

By signing this release and waiver of liability (the “Release”), you do so in favor of Monkey Rock, Inc., its directors, officers, administrators, employees, and agents (collectively referred to herein as “Monkey Rock”) and give up the right to sue Monkey Rock for any injury or damages, of whatever kind or nature, that may arise from or relate to your participation in the programs and activities offered by Monkey Rock (the “Monkey Rock Activities”).

I, hereby sign this Release on behalf of myself, my personal representatives, heirs, next of kin and anyone who obtains rights from me, and on behalf of my child(ren).

I desire to participate in the Monkey Rock Activities and understand that such participation may involve risk of bodily harm, serious injury, death, or personal property damage or loss. For this reason, I do hereby freely, voluntarily, and without duress execute this Release under the following terms:

1. Waiver and Release. I do hereby release and forever discharge and hold harmless Monkey Rock and its successors and assigns from any and all fault, negligence, liabilities, costs, claims, actions, demands, penalties, losses, lawsuits, arbitrations, or damages of whatever kind or nature, either in law or in equity, which arise or may hereafter arise, in any way, from my participation in the Monkey Rock Activities.

2. Medical Treatment. I do hereby release and forever discharge Monkey Rock from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my participation in the Monkey Rock Activities.

3. Assumption of Risk. I do hereby expressly and specifically acknowledge and assume the risks inherent in the Monkey Rock Activities and release Monkey Rock from all liability for harm, injury, illness, death, or damage to personal property that arises from or relates in any way to my participation in the Monkey Rock Activities.

4. Photographic Release. I do hereby consent to the use of my likeness in print, photographic, digital, electronic, or social media by Monkey Rock in connection with Monkey Rock’s marketing, advertising, and promotion of the Monkey Rock Activities and the Monkey Rock brand. I give Monkey Rock permission to use my personal information for marketing and promotional purposes, including, but not limited, to newsletters, emails text offers, discounts and promotions.

5. Scope. I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of Louisiana and that this Release shall be governed by and interpreted in accordance with the laws of the State of Louisiana that in the event any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release, which shall continue to be enforceable.

I have read and fully understand this Release and that I am assuming all risks inherent in participation in the Monkey Rock Activities. I voluntarily sign my name, and thereby state my acceptance of the above provisions.
