$10,000 Hole-in-One Challenge Waiver Form

 

Terms and Conditions:

By signing this form, you acknowledge and agree to the following:

1. Eligibility:

  • Participants under 18 must have a parent or guardian (18+) sign this waiver on their behalf.
  • All golf balls used must be purchased at Cattail Crossing specifically for use in the Hole-in-One Challenge.
  • Participants must be amateur golfers and cannot hold any PGA, LPGA, or worldwide professional golf status.
  • Participants must not be employed by, or be a direct relative (parent, sibling, spouse, child) of, any Cattail Crossing Golf & Winter Club staff member.
  • Participants are not allowed to practice on the driving range within 30 minutes of attempting the Hole in One Challenge.

2. Hole-in-One Verification:

  • All potential hole-in-ones will be verified by video footage before any prize is awarded. Distance must be a minimum of 100 yards.
  • Any hole-in-one not clearly captured by video footage or otherwise verifiable will be deemed ineligible for the prize.

3. Use of Name and Likeness:

  • By participating, you agree to allow Cattail Crossing Golf & Winter Club to use your name, photograph, video, and likeness for advertising, promotional purposes, and social media posts without any additional compensation.

4. Acknowledgement of Risk:

  • Participants understand that golf is a sport involving inherent risks and accept full responsibility for any injury, loss, or damage resulting from participation in the event.

5. Release of Liability:

  • In consideration for participation, I hereby release and hold harmless Cattail Crossing Golf & Winter Club, its owners, managers, employees, and agents from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) sustained during the event.
