LIABILITY WAIVER

In consideration of being allowed to participate in any way in TX HOP Club program, games, tournaments, events, and activities, I acknowledge and agree to the following:

ASSUMPTION OF RISK

I fully understand and acknowledge that my participation in the TX HOP Club Activities have inherent risks and hazards, including but not limited to property damage, personal injuries, disability, fractures, death or other serious illness. I confirm and acknowledge that my participation in the TX HOP Club Activities is voluntary, and the instructors clearly explained to me the risks, the benefits, and the procedures for each activity.

FACILITY RULES AND REGULATIONS

I understand and consent to be bound by the rules and policies explained by the TX HOP Club in its Code of

Conduct (Posted in facility). In any case that the TX HOP Club gets hold of competent proof of my violation of its Code of Conduct, negligently or intentionally, it has the right to expel and disqualify me from participating in the club’s activities and events. TX HOP Club reserves the right to terminate the membership of members who are found to be in breach of the Code of Conduct.

COVENANT NOT TO SUE

I hereby discharge and promise not to sue the TX HOP Club and its authorized agents from any liabilities and claims as a result of my use of the facilities, equipment or participation in authorized activities.

INDEMNIFICATION

I acknowledge and consent to indemnify the TX HOP Club against any legal liabilities as a result of my participation in its authorized activities.

REPRESENTATION

I acknowledge and represent that I am over the age of 18 and of sound mind and body.

GOVERNING LAW

This Waiver governed by the laws of City of Granbury, Hood County, Texas.
