MEMBERSHIP TERMS AND CONDITIONS
As a member, you shall be entitled to tan up to a maximum of once per day with any of the chosen packages. Equipment and maximum tan times will vary. The purchaser is entitled to the below chosen monthly rate unless the purchaser discontinues the monthly payments. Upon discontinuance of the monthly payments this agreement is considered null and void. The purchaser is not entitled to the below monthly rate unless there has been at least 1 automatic payment. The purchaser may choose to "freeze" their account for a maximum of 2 months for $9.99 per month. Notice of the intent to freeze needs to be given one month in advance. This option is limited to once per contract year. This will allow the member to keep their membership 'active' and avoid paying the enrollment fee to re-start the plan. The $9.99 fee per month is a Pre-Authorized Payment. Once the designated freeze period expires, the monthly payments will be re-instated automatically. Cancellation or freezing notice must be given in person, and you must obtain your 'Proof of Cancellation/Freezing Notice' from the staff member. This will protect you in the unlikely event that your monthly payment is not stopped according to your instructions. Please keep your copy of the cancellation slip until your payments have stopped. The purchaser understands that only Pre-Authorized Payments can be accepted on the first of each month. The purchaser has the option of Pre-authorized Visa/MasterCard, or a Pre-authorized bank withdrawal. Monthly cash payments, post-dated cheques, or any in store payment cannot be accepted. The purchaser is only entitled to the below rate if the payments are on a Pre-Authorized Payment Plan. Any changes to banking information must be given one month in advance. Preauthorized payment information must be given to Sabrina's Salon Corporation DBA Solstice Tanning & Beauty, or the membership will be suspended. If for any reason the purchaser wishes to opt out of the monthly payment program, you may do so at any time providing you have given Sabrina's Salon Corporation DBA Solstice Tanning & Beauty written notice at least one full calendar month prior to the final pre-authorized payment. For example, if you wish your last payment to be March 1, then written notice must be received by Feb 1. If no cancellation is given, the automatic payments will continue. Your account must be in clear standing and no dues owing before cancellation will take effect. Failure on your part to take advantage of the privileges of membership shall not relieve you from liability for dues.  The purchaser understands that this agreement is non-refundable and non-transferable to another person. The purchaser will be given 30 days notice prior to any membership pricing increases. Notice to Purchaser: Do not sign this agreement until you have read it and have determined that there are no promises, terms, or conditions, whether verbal, written or implied, other than those written herein. I/we understand hereby authorize Sabrina's Salon Corporation DBA Solstice Tanning & Beauty to debit my/ our bank or credit card account for all the pre- authorized monthly fees. There is a $25.00 collection charge for any payment, cheque or draft which is returned, not honoured or stopped. January 12, 2025 |
