Membership Terms:

Term/Fulfillment Date: Membership pricing is based on a 24 month term that may be cancelled at any time with just 30 days written notice and a $50 early cancellation fee (see cancellation).

Monthly Payments: Payments are due on the 1st of each month and payable through automatic EFT. I authorize automatic payments from the bank account or credit card provided below. Returned or declined payments will result in a $25 service fee in addition to any charges from your bank.

Cancellation: Members may cancel at any time by completing the Cancellation 30-day Notice Form in writing at the home spa. Memberships will automatically renew on a month to month basis until cancelled by the member. The early cancellation fee is payable in spa only if the 24 month term has not been completed, otherwise just the Cancellation 30-day Notice Form is needed.

• UV Tanning is limited to once every 24 hours to prevent overexposure.

• Heat Therapy Services are limited to 1 heat therapy service per day.

• Red Light therapy sessions are limited to every 48 hrs.

Membership Holds: Memberships may be placed on hold for $10/month for up to 3 consecutive months. To place your membership hold, complete the Membership Hold Form before the 23rd of the month in order to process the request for the next billing cycle periods that do not count towards the 24 month term and memberships may not be cancelled while on hold.

Membership Changes: I understand that all changes to my membership billing (holds, upgrades, downgrades, transfers and cancellations) must be done in writing. Verbal requests do not create the necessary documentation to ensure billing accuracy. Please complete the form request and keep a copy for your records.

Service Appointments: I understand that if I book appointments and do not show up or provide 2 hours cancellation notice I will be charged $5 per service appointment missed for both late cancellations and no shows.

Products: Products are not included in monthly membership dues. I understand some services may require additional product purchase for example: tanning goggles, tanning lotion, red light products, teeth whitening, etc.

Use of Services: I understand and agree to the monthly payments for my membership and it is up to me to attend and regularly utilize spa services, Refunds/credits will not be issued in the event of non-use.

Membership Rules: Member may use all of the equipment available according to the membership type purchased, which is indicated in this agreement. By signing this agreement, member agrees to abide by all Automated Spa rules and regulations now in effect or to become effective at any future date. All rules, regulations and services are subject to change without notice. Memberships are non-refundable/ Automated Spa is not responsible for lost or stolen goods. Photo ID and/or membership card must be presented upon entry. All personal items are the sole responsibility of the member (keys, wallet, etc.) Abuse of equipment or staff will not be tolerated. Violation of Automated Spa rules may result in membership termination.


Liability Waiver: Member expressly agrees that all use of the Automated Spa facilities and equipment shall be at member's sole risk. The location group shall not be liable to member and member shall hold harmless location group from and against any and all claims, suits or demands, including, but not limited to those for bodily injury, illness, disease or death sustained by member and/or its guest or property loss or damage of member and/or its guest, arising out of or in any way connected with the services provided by location group, regardless of whether or not any such claim, suit or demand shall arise in whole or in part from the sole or concurrent negligence, omission or fault of any member of location group, or strict liability or product liability. The location group shall not be responsible or liable to member and/or its guest for articles lost or stolen in the spa or for loss or damage to any other property of member and/or its guest, including their automobiles and contents. The term location group, as used herein, shall mean each Planet Beach Automated Spa used by member and/or its guest, Planet Beach Franchising Corporation, and their owners, directors, officers, agents, employees, insurers, successors, assigns, parents, subsidiaries and affiliated companies, guests and invitees.

Parent(s) or Court-Appointed Legal Guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms of this document, as set forth above.
