Furfilled Pet Services Waiver and Release Form

Emergency Veterinary Care

I agree that in the event that my pet appears ill, injured, or is exhibiting any other behaviour that would suggest my pet might require medical treatment, if attempts by Furfilled Pet Services to contact me are unsuccessful, Furfilled Pet Services may obtain veterinary care for my pet(s). I agree that Furfilled Pet Services may obtain veterinary care from any veterinary hospital that is able to see my pet(s). I agree that I am fully responsible for any and all costs in connection with veterinary treatment.

 



Media Release

I agree to allow Furfilled Pet Services to use my pet's name and any images or videos taken while they are in Furfilled Pet Services’ care, in any form, for use at any time, in any media, marketing, advertising or promotional materials.

Notice must be given at least 24 hours in advance of services or payment in full will be due.


 

LiabilityWaiverandRelease

I understand that by signing this waiver, I willingly agree to assume complete responsibility for any injury, personal property damage, loss/theft, or death that may occur while Furfilled Pet Services is watching my pet(s). I waive all claims for damages, injuries, diseases, loss/theft, and death sustained to my pet(s) or my property that I may have against Furfilled Pet Services relating to the pet care. I understand that my pet could be lost or escape, be injured, or die while being left in care with a pet care professional, and that my home, property, or personal belongings could be damaged or stolen during this time. By this waiver, I assume any risk, take full responsibility and waive any and all claims of damages to personal property, injuries, diseases, loss/theft, or death pertaining to my pet(s) associated with Furfilled Pet Services.

I understand that while Furfilled Pet Services carefully monitors and supervises all pets while in their care, animal behaviour can be unpredictable and illnesses not apparent. I understand that due to the way that animals interact with one another, minor cuts and scratches can occur even though the animals are carefully supervised at all times. I hereby release Furfilled Pet Services from any and all liability and claims of which arise or may hereafter arise from the services provided by Furfilled Pet Services.

I agree that Furfilled Pet Services provides reasonable supervision and care and may not detect fleas or ticks on my pet(s). I understand that I therefore may not be notified if my pet(s) contracts fleas or ticks.

I understand that Furfilled Pet Services and their employees are not veterinarians and cannot diagnose or detect illnesses in pets. I agree to assume all risks associated with administration of medication by Furfilled Pet Services.

I agree to hold Furfilled Pet Services, its owners, directors, officers, employees or agents harmless in the event of injury or death during transportation of my pet(s).

I understand that I am solely responsible for any harm caused by my pet(s) whilst they are in the care of Furfilled Pet Services. I agree to be financially responsible for any damages or injury that my pet may cause to other animals, humans, personal property and/or Furfilled Pet Services. I agree that if my pet is injured by another pet, I hereby release Furfilled Pet Services, its owners, employees and agents from all liability and financial responsibility for such injury.

I agree that I have disclosed and will continue to disclose any and all medical, behavioural, or other conditions that may affect, limit, or prevent my pet’s ability to participate in the services provided by Furfilled Pet Services. I understand that Furfilled Pet Services relies on this information to provide a safe environment for both humans and animals.

I have read and fully understand the terms of this agreement and understand that I will give up substantial rights by signing it. I have signed this agreement freely and voluntarily without any inducement, assurance or guarantee and intend it to be a complete and unconditional release of all liability Furfilled Pet Services, officers, directors, employees, and agents to the greatest extent permitted by law. I further agree that if any portion of this agreement is held to be invalid or unenforceable, the remainder of this agreement shall remain in full force and effect.
