Permanent jewelry is a process that involves welding the jewelry together while on your body. The procedure is generally painless and performed with your safety as a top priority. This consent form is intended to inform you of the potential risks of the procedure and to obtain your informed consent to participate.

7-Day Quality Guarantee

Bella Elegante offers a 7-Day Quality Guarantee. If your jewelry detaches at the weld site within 7 days of your appointment, you may return to the studio for one free re-weld. In certain circumstances, a refund may be offered at Bella Elegante’s discretion. After 7 days, a re-weld fee will apply. This guarantee applies only to breakage at the weld site; it does not cover lost, stolen, or damaged chains.

Acknowledgments and Consent

  • I understand that while the jewelry is described as "permanent," it is not indestructible. Reasonable care is required to maintain its integrity.
  • I understand that in rare cases, allergic reactions or skin irritation may occur. Bella Elegante is not responsible for these reactions.
  • I understand that in the event of an emergency, medical procedure (such as an MRI), or personal choice, the jewelry can be safely removed with scissors or wire cutters.
  • I understand that all sales are final due to the custom nature of the jewelry.
  • I understand that Bella Elegante is not responsible for lost, damaged, or stolen jewelry.
  • I understand that the welding process carries some risks, including but not limited to: allergic reactions, skin irritation, discomfort, minor burns, and possible scarring. I voluntarily accept these risks and assume all responsibility for any consequences that may arise from the procedure.
  • I agree to follow all aftercare instructions and recommendations provided by the technician to help ensure the longevity of my jewelry.
  • I understand and agree that if I believe conditions are unsafe, or if I feel unwell during the procedure, I will immediately inform the technician and discontinue participation.
  • I authorize Bella Elegante to provide or arrange emergency medical care if necessary during the procedure. I understand that I am financially responsible for any medical treatment or transport and hereby release Bella Elegante from any claims arising from such services.

I give Bella Elegante permission to photograph or record the procedure and my jewelry for promotional or marketing purposes.

BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTOOD, AND AGREE TO THE TERMS OF THIS WAIVER. I UNDERSTAND THAT I AM VOLUNTARILY GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE BELLA ELEGANTE. ALL SALES ARE FINAL.
