Transportation Waiver

Transportation Consent Form

(Applies to all traveling and event transportation) 

I give permission for myself, and/or my youth, to be transported to and from Troy Skate Church - 3LC Ministries Inc sponsored activities in a church, rental, or private vehicle. I understand that I, and/or my youth, are expected to follow all applicable laws regarding riding in a motor vehicle and are expected to follow the directions provided by the driver and/or other Troy Skate Church - 3LC Ministries Inc representatives and volunteers. 


Insurance and Personal Belongings Release

I realize Troy Skate Church - 3LC Ministries Inc insurance begins where the individual health and accident policy terminates. It is only valid when all other insurance has been extended to its limits. In addition, I realize that Troy Skate Church - 3LC Ministries Inc, its officers, directors, employees, agents, any other parties volunteering on behalf of the church, or its sponsors are not responsible for personal belongings.


Photo Release

Additionally, I grant Troy Skate Church - 3LC Ministries, Inc permission to use my likeness or my minor's likeness in a photograph or video in any and all publications including but not limited to any of Troy Skate Church - 3LC Ministries, Inc's printed and digital publications. I understand that the photograph or video is property of Troy Skate Church - 3LC Ministries, Inc and I am granted no royalties, financial compensation, or rights to them. I authorize Troy Skate Church - 3LC Ministries, Inc and their employees and agents to edit, alter, copy, exhibit, publish, or distribute photos or videos for the purpose of publicizing Troy Skate Church - 3LC Ministries, Inc's programs or for any other related, lawful purpose. 


General Release

The undersigned, guardian, or a member of the immediate family of the undersigned realizes that the participant may incur personal injury or bodily damage while participating in activities and transportation, and acknowledge that Troy Skate Church - 3LC Ministries Inc, its officers, directors, employees, agents, or any other parties volunteering on behalf of the church, shall be held harmless from all actions, claims, costs, expenses or damages of any kind, growing out of or relating to any activities of the organization. The undersigned, guardian, or a member of the immediate family of the undersigned further acknowledges this is the full and complete release for all injuries and damages which the participant may sustain as a result of participating in any activity. 

I, being the legal guardian or self, give permission for the participant to be transported to and from Troy Skate Church - 3LC Ministries Inc sponsored activities. Therefore, I assume all risks associated with Troy Skate Church - 3LC Ministries Inc, even if they arise from the negligence of the Troy Skate Church - 3LC Ministries Inc, promoters, officials, and volunteers, transporting myself, and/or my child. My, and/or my child's, participation in this activity is voluntary and no one is forcing me, and/or my child to participate. I understand the effect of this waiver and acceptance of risk on my legal rights.

By signing this transportation waiver, I hereby fully and forever release and discharge indemnify and hold harmless the Troy Skate Church - 3LC Ministries, Inc and their employees, agents, and volunteers from any and all liabilities, claims, demands, damages, rights of action, suits or causes of action present of future, whether they same be known or unknown, anticipated or unanticipated from the transportation of myself, and or my child.
