Kids Camp Sign Up and Liability Waiver


Policies and Procedures

● For the well being of the other children, please refrain from bringing a sick child into the

childcare area. (See Liability Waiver on next page for more details).

● Please label all of your child’s belongings with his/her name.

● Continual disruptive or inappropriate behavior will not be tolerated and the parents will be notified and potentially required to remove the child from participation.

● Please pick up your child promptly once camp is finished.

● Children will be released ONLY to the person who dropped them off. Parents MUST sign each child out before leaving the building. You may leave the premises while child is participting in camp, but a contact number must be provided.

● Our team will not administer medications, will not accompany to the bathroom. If parent/guardian is leaving the facility, child must be bathroom sufficient on their own.

Liability Waiver

I, the undersigned, individually and as the parent of my child/children, hereby hold harmless, waive and release Unsettled LLC, their childwatch workers, officers, representatives, agents, organizers, and successors (“Unsettled”) from any and all liability arising out of or as a result of any personal injury or property damage my child/children may suffer while my child/children is/are in the facility at Unsettled, LLC / Burn Boot Camp | Layton, UT. I fully understand and acknowledge that Unsettled, LLC kids camp services are provided at my permission and to provide activities for my child[ren] not as a certified educational facility. I further understand and acknowledge that Unsettled LLC/Burn Boot Camp, is a “drop-in” daycare facility and is NOT required to be licensed OR regulated by the State of Utah. I understand that I am NOT to bring a sick child to the daycare; this includes but is not limited to, any signs of fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, loss of taste or smell, congestion or runny nose, nausea or vomiting, diarrhea, earache, discharge from the eye, or open sores.

I understand that while participating in this activity, my child may be photographed. I agree to allow their photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns. 

I understand that while attending kids camp, my child will receive the snack I provide for them.

Additionally, I understand and acknowledge, that the person(s) monitoring my child may not be childcare experts or professionals, and may in fact be volunteers with no special child care training or experience. By dropping my child/children off at Unsettled LLC/Burn Boot Camp’s kids camp, I understand and consent to this fact. I also understand that even with reasonable monitoring, my child may, in the course of play or as a result of simply sharing a semi-public space with others, suffer an injury or harm, including without limitation broken bones, strains, sprains, bruises, concussions, viral, bacterial, or other pathogen infections, sickness, or illness, disability, or even death. I acknowledge and agree that these injuries or harms are risks inherent in child’s play, and I assume the risk of any injury or harm, regardless severity that my child may sustain as a result of the use of the Unsettled LLC/Burn Boot Camp’s kids camp. I hereby waive any claim I, individually, or as a parent, may have against Unsettled LLC/Burn Boot Camp as a result of any such injury or harm.


I understand and acknowledge that this Agreement shall be governed by the State laws of Utah, and irrevocably consent to venue in a state court in Davis County, Utah. I further understand and acknowledge that by signing below I am waiving any objection to venue in Davis County, and to my right to request a jury trial.

I hereby agree that in no event shall Unsettled LLC/Burn Boot Camp be liable for any cause of action, claim, damage, demand, expense, fine, investigation, liability, or penalty arising out of the kids camp contemplated under this Agreement for any amount in excess of the total amount that I paid to Unsettled LLC/Burn Boot Camp in the preceding 12 months.

I further agree to defend, hold harmless, and indemnify Unsettled LLC/Burn Boot Camp for any damages, expenses, court costs, attorney fees, and/or liabilities of any sort caused or claimed to be caused by the negligent or reckless acts or omissions or intentional acts or omissions of my child/children.

In consideration of my application and permitting my child to participate in this activity, I hereby: WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of Unsettled LLC/Burn Boot Camp, its trustees, officers, employees, camp counselors, volunteers, entities or other persons released, for my child’s death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to them including their traveling to and from this activity;

This Agreement, along with any other written, executed, and valid agreement between the Parties, constitute the entire agreement of the Parties, and supersede all prior oral agreements, and/or memoranda. This Agreement may only be modified in writing with the written consent of both Parties. This Agreement is intended to bind only the Parties hereto, and their successors, and may not be assigned by either Party without the express written consent of the other.

In the event that a court determines that any provision of this Agreement is invalid, all other provisions shall survive.
