Mountain Sports School Lesson Release Form

Release of Liability

As the parent or legal guardian or other legal representation of said minor child, I understand and am aware that there are inherent and other risks involved in participating in ski and snowboard lessons which could cause injury to the child.

I hereby release the Snowy Range School, their representatives, employees, agents, officers, and directors from any and all liabilities, claims, and causes of action of any kind or nature which may occur to said minor child arising out of participation in the Snow School Program, and indemnify and hold them harmless from liability, claims, and causes of action of any kind or nature for any loss, damage, injury, illness, or harm of any kind or nature to any person or property which is caused or contributed by said minor child, whether alone or in concert with other minor children. Should I elect not to outfit my child with a safety helmet, I understand that this release applies to anything that might occur as a result of my failure to provide such a helmet. I also understand that said lesson(s) may progress to a level where the students are ready to ride a chairlift. It is not guaranteed that all students will ride the lift with an instructor. Students may be riding the lift with; other students, chaperones, general public, or alone. It is the students responsibility to ride the lift safely and if this cannot be done the minor may be expelled from the lesson.

In the event of a medical emergency, relative to said minor child, I hereby authorize Snowy Range Snow School, Snowy Range Ski and Recreation Area, their representatives, employees, officers, and directors to take such responsible action to care for such minor child, in addition to calling for medical care and emergency services, as is necessary or helpful in their opinion, and hold them harmless on account of any such action.

I understand that group lessons end at 12:00pm or 3:30pm or depending on the type of lesson, or if otherwise stated by the director or instructor at the start of each lesson, and I am to pick up the said minor child at that time. I understand that I alone may pick up said minor child at the end of the lesson unless I designate another person(s) in the space below who will then assume responsibility for the child upon the presentation of proper photo identification.
