Northwest Ninja Park Adult Waiver

I acknowledge that I am voluntarily participating in one or more fitness classes, open gym sessions, and/or other fitness-related activities (collectively, “Fitness Activities”) offered by Northwest Ninja Park, LLC, an Oregon limited liability company (the “Company”). 

 

            I represent that I am in good health and that I assume all risks connected with participation in the Company’s Fitness Activities.  I understand that participation in any exercise or fitness activity is inherently risky.  I agree to disclose to the Company any condition or physical limitation, disability, ailment, or impairment that may affect my ability to participate in any of the Company’s Fitness Activities. 

 

             In consideration for being allowed to participate in the Company’s Fitness Activities, I hereby release and indemnify the Company and its members, managers, agents, representatives, and employees from and against all actions, claims, demands, losses, expenses, and attorneys’ fees for any property damage, personal injury, or death I may suffer in connection with my participation in the Company’s Fitness Activities (except to the extent caused by the Company’s intentional misconduct).  This waiver and release is intended to be binding on my spouse, heirs, and the personal representative of my estate.  I have carefully read and understand the terms of this waiver and release, and I am signing it freely and voluntarily.
