New Client Consent Form “Tanning”

You have agreed to participate in an airbrush tanning session to achieve a “sunless” tan on your skin. To help you best achieve your desired results, please read the following material carefully and acknowledge your understanding and agreement by signing below.

1. Airbrush tanning is accomplished by application of a solution containing the active ingredient Dihydroxyacetone (DHA). DHA is considered to be safe and has been FDA approved in cosmetic use.

  • It does not provide any UV protection. Please use SPF as usual
  • The FDA recommends that all users of DHA spray tanning services avoid inhaling or ingesting DHA. It is recommended that you use protective undergarments, eyewear, nose filters and seal lips with balm prior to your service. All protective measures are available from your technician.

2. If you have sensitive skin or allergies to sunless tanning products containing DHA, or if you are unsure whether you may be allergic to DHA, we strongly suggest consulting your physician prior to tanning.

  • Allergic reactions such as minor skin rash are extremely rare, but possible. If you have never used a sunless tanning product, we suggest a patch test 24 hour before the full body application. Honeysliice LLC is not responsible for any allergic reactions that may occur after or during use.

3. If you have a history of asthma or other respiratory or medical conditions it could be aggravated by the use of the airbrush tanning process. Please consult a physician before proceeding.

  • Please tell your tanning technician about any past allergic reactions with DHA products or other medical concerns. Honeysliice LLC is not responsible for the aggravation of any medical conditions that may occur after or during use.

4. Some of the ingredients in our solution contain a sugar base and application can cause a rise in blood sugar levels. If you are diabetic or have medical conditions that can be affected or aggravated by a rise in blood sugar, please consult your physician before use.

5. The bronzing and tanning solutions used during your session have the potential to stain clothing. (But not usually) We recommend that you wear something dark in color that can be stained without concern, such as an old swimsuit or undergarment. Some clients purchase an inexpensive swimsuit dedicated to their tanning sessions. For your convenience, disposable bikinis which create minimal tan lines are available upon your request. After your tanning session, you should wear dark, loose fitting clothes.

  • It is also possible that the solutions may transfer to other clothing, furniture or car seats after your session. Washing or dry cleaning will usually, but not always, remove the color.
  • We suggest bringing a towel to cover your car seat. Honeysliice LLC is not responsible for potential stains to clothing or upholstery. Please be conscientious and careful.

6. Your tanning session will take approximately 15 to 20 minutes. After spraying, your skin should be dry before putting your clothes back on and you should not bathe, shower or sweat excessively for 1-6 hours (Rapid) 8 to 10 hours(Overnight). Exposure to excessive moisture before this time period may affect your results. When your session is over the solution will give you an immediate bronzing effect. When you bathe, the bronzer will come off.

Client Conditions:

1. Honeysliice LLC will not be responsible for lost, stolen, or damaged personal items.

2.  The airbrush tanning process has been explained to me and I hereby agree to allow Honeysliice LLC to apply spray tan solution to my skin using the airbrush tanning process.

3.  To my knowledge, I have no medical condition or allergy which would preclude me from having this procedure done.

4.  I agree to hold harmless Honeysliice LLC of any medical complications that may arise and I have been advised to discontinue use if any reaction occurs.

5.  I understand that the results of airbrush tanning do vary and that no guarantees of specific results are offered or implied.

6.  I hereby release this establishment, as well as the airbrush technician, and hold them harmless from any liability involved in the use of or arising during the tanning process.

7.  I also agree to first contact Honeysliice LLC if for any reason I am dissatisfied with my results.

No Extra Guests:

Yes this includes kiddos

  1. These appointments can be long and having someone else in the room (especially children) can be very hectic, distracting and potentially dangerous for kids because I/ you cannot watch them.
  2. My insurance only covers myself and the current client

If you choose to bring an extra guest, no matter the age, I will have to reschedule your appointment and charge a 100% fee due to not being able to fill the appointment slot. I understand that finding child care can be difficult but ultimately it is for everyone's safety. There is a fantastic daycare in the area called Kiddin' Around with great reviews and they CHARGE BY THE HOUR - perfect for dropping kiddos off for an appointment (There are some initial forms to fill out so you will need to set that up ahead of time)

Cancellations:

24 Hours prior

If a 24 hour notice is not given you will incur a fee equivalent to 100% of the booked appointment

If you cancel within one hour of your appointment or simply do not show up, you will incur a fee of 100% of the booked appointment

This also includes multiple services - any changes to an appointment must be made 24 hours in advance so if you decide you don't want part of your service you will still be charged for it due to the time being taken up on the schedule

As cancellations/ no shows have a significant impact on me and my business, I need to enforce my cancellation policy and trust you understand my need to do so.

 


Timely Arrival

Your appointment time is set just for you, so if you are late you get what time is left - every minute missed is a significant amount of lashes that aren't being filled. This will also potentially give you worse retention as I am unable to fill all of your lashes. There will be no "fixes" given if the client is late to the appointment.

No discounts will be given for late appointments (Ex: If you are scheduled for a Hybrid Fill with a Time Add On and you show up in enough time for the regular fill you will still be charged for The Fill and the Add On)

Certain services don't have wiggle room with time so if you are running late you may have to be rescheduled

If you need to use the restroom please show up early to do so!



Refunds

No refunds will be given if you have any issues please reach out so we can address any concerns


Card on file

When entering a card to keep on file I Lauren Taylor Owner of Honeysliice LLC have no knowledge as to which card is being entered and am not responsible for unauthorized use of the card. If you choose to use a card that is not your own and a charge back is filed you are agreeing to take responsibility for this action and could potentially have fraudulent charges brought against you by the card owners credit card company.
