BOXFIT808, LLC Waiver, Consent, Release and Indemnity Form

As the parent or legal guardian of the child or ward named above, I hereby give my full consent and approval for myself, my child or ward to join BOXFIT808, LLC; BOXFIT808 as a club member and to participate in the sport of boxing, including but not limited to exercising, training, sparring, boxing matches and travel.

In consideration of myself, my child or ward’s membership and participation in BOXFIT808, LLC; BOXFIT808 and any related activities thereto, including but not limited to exercising, training, sparring, boxing matches and travel associated with BOXFIT808, LLC; BOXFIT808, on behalf of myself, my child/or ward and agents, heirs, assigns, executors, administrators, relatives, associates, personal representatives, settlors, trustees, beneficiaries, successors and next of kin, I hereby agree to waive, release and covenant not to sue BOXFIT808, LLC; BOXFIT808, Phillip Ramirez, both individually and in their official capacities, and their coaches, officers, directors, associates, employees, volunteers, staff, agents, heirs, assigns, executors, administrators, relatives, personal representatives, settlors, trustees, beneficiaries, successors, next of kin and/or sponsors (hereinafter “Released Parties”) for any and all liabilities, claims, injuries, damages, actions, rights, demands, costs or expenses of any nature, arising out of or in any way connected with me or my child or ward’s membership in BOXFIT808, LLC; BOXFIT808 and/or participation in BOXFIT808, LLC; BOXFIT808 related activities.

On behalf of myself, my child or ward, I further agree to indemnify and hold each of the Released Parties harmless from and against any and all such liabilities, claims, injuries, damages, actions, rights, demands, costs or expenses of any nature, including but not limited to any and all attorneys’ fees and disbursements up through and including any appeal. I understand that this waiver, release and indemnity includes any claims based on the negligence, action or inaction of any of the Released Parties and covers any bodily injury (including death), property damage, and loss by theft or otherwise, whether suffered by me, my child or ward either before, during or after such membership or participation.

I agree to abide by the BOXFIT808, LLC; BOXFIT808 Rules and Code of Conduct and by the Rules of USA Boxing, and agree to follow the USA Boxing Part II Constitution & By-Laws Article XIV Complaint Procedures to resolve any substantive issues, disagreements or rule violations.

I fully understand and assume all responsibility and liability for any injury or damage that I, my child or ward may incur during training, sparring, boxing competitions or other BOXFIT808, LLC; BOXFIT808 related activities. I understand and agree that medical or other services rendered to me, my child or ward by or at the insistence of any of the Released Parties are not an admission of liability to provide or continue to provide such services and is not a waiver by any of said parties of any right(s) hereunder.

I certify that I, my child or ward do not have any injuries, health or physical conditions that prevent membership and/or participation in any BOXFIT808, LLC; BOXFIT80 activity, including but not limited to exercising, physical training, sparring and boxing matches. I also understand and appreciate that participation in exercise, training, sparring, boxing matches and other related BOXFIT808, LLC; BOXFIT808 activities involves risks of serious injury to me, my child or ward, including but not limited to permanent disability, serious bodily injury, paralysis, brain injury, blindness, concussions and even death. I voluntarily and knowingly recognize, accept, and assume these risks on behalf of myself, my child or ward.

I hereby certify that I, my child or ward is fully capable of participating in exercise, training, sparring and boxing matches and that I, my child or ward is healthy and have no physical or mental disabilities or infirmities that would restrict full participation in these related activities.

I authorize the Released Parties in attendance at any BOXFIT808, LLC; BOXFIT808 activity to select and secure medical attention at my cost, as may be necessary for me, my child or ward as a result of injuries or other events requiring emergency care while I/we are not in attendance at such events. I hereby release said Released parties from any and all liability on account of such selection or authorization for any and all damages which occur on account thereof.

I further understand that I, my child or ward may only exercise, train, spar, work out or box in a match as a BOXFIT808, LLC; BOXFIT808 club member when there is an authorized BOXFIT808, LLC; BOXFIT808 coach physically present in the gym who authorizes the activity

and is directly supervising the exercise, training, sparring, work out or boxing session. I agree that I, my child or ward will not under any circumstances, exercise, train, spar, work out or engage in any boxing or other BOXFIT808, LLC; BOXFIT808 related activity unless an authorized BOXFIT808, LLC; BOXFIT808 coach is physically present at the location and is directly supervising the exercise, training, sparring, work out activity or boxing match. If I, my child or ward engage in any exercise, training, sparring, work out activity or boxing match outside the physical presence of a BOXFIT808, LLC; BOXFIT808 coach, I, my child or ward fully and completely understand that such activity is independent of and not related in anyway to BOXFIT808, LLC; BOXFIT808.

I, my child or ward has familiarized ourselves with the BOXFIT808, LLC; BOXFIT808 rules and Code of Conduct, Rules of USA Boxing, and with the risks inherent in boxing. My understanding and acceptance of them, as signified by my signature below, is a condition of my membership in BOXFIT808, LLC; BOXFIT808.

I, my child or ward agree to abide by BOXFIT808, LLC; BOXFIT808 Rules, Code of Conduct, Rules of USA Boxing and any other rules and policies that relate to any activities associated with the BOXFIT808, LLC; BOXFIT808.

If I, my child or ward observe or feel there is any unsafe condition/situation, risk or other hazard posed during my membership and/or participation in a BOXFIT808, LLC; BOXFIT808 related activity, including but not limited to exercising, training, sparring, working out or boxing, or otherwise feel that they are unable to participate in any BOXFIT808, LLC; BOXFIT808 related activity or event, they will immediately refuse to participate and will remove themselves from participation in the activity and/or event. They will also immediately notify a BOXFIT808, LLC; BOXFIT808 coach and/or representative of any such condition, risk or hazard.

REMINDERS: (please initial after each statement)

If you have started AFTER the First Week of the month, we have pro-rated your tuition, so your FULL Tuition is DUE the FIRST Week of the month.


Tuition is for the month. It is your responsibility to make-up the day you missed during that month. 


PRIVATE LESSONS: You must call/text/email confirmation the NIGHT BEFORE your session AND at least 1 hour before your session. IF THERE IS NO CALL/TEXT/EMAIL the NIGHT BEFORE, there is no guarantee that your lesson will be held. IF THERE IS A CALL/TEXT/EMAIL the NIGHT BEFORE but CANCELLED/NO SHOW THE DAY OF THE SESSION, you will be charged for that session. 


You must arrive by 10 minutes after the hour for the BOXING CLASSES otherwise; you must wait until the next class or OPEN GYM.
