WAIVER FOR GUEST REGISTER / SIGN IN SHEET 

By signing this waiver, the members (s) or guest(s) agrees that he or she undertakes the sole responsibility for themselves while attending Big Tex Gym/ Eon Fitnezz LLC. You voluntarily assume all risks of accident or damage to yourself or your property and loss thereof. It is acknowledging that Big Tex Gym/ Eon Fitnezz LLC does not provide patrolling in the parking areas that a person may use when attending Big Tex Gym/ Eon Fitnezz LLC. By signing, you are assuming the risk of yourself and those that came with you to Big Tex Gym/ Eon Fitnezz LLC. You’re attending the gym with the understand that you are in good health and that you have no conditions that could be aggravated or worsened by the use of the gym, gym facilities, or equipment. You will be responsible for re-racking weights, put back any equipment, and wipe down after every machine useage with provdied paper towels and disinfectant spray from our cleaning station, any violation of rules mentioned in this waiver will result in $100 fine or more. Big Tex Gym/Eon Fitnezz LLC reserve the right to refuse service to anyone. In no event shall Big Tex Gym/ Eon Fitnezz LLC nor its officers, agents, employees be liable for any incidental or consequential liability damage or demand of any kind whatsoever relating to the usage of member(s) or guest(s) of the facilities and equipment of Big Tex Gym/ Eon Fitnezz LLC.
