WADE’S EASTSIDE GUNS

CUSTOMER SAFETY & RENTAL AGREEMENT

Anyone entering the range, including observers, MUST completely read and fill out this form. All applicable areas of the form must be individually CHECKED, SIGNED and COMPLETED by the actual person. All individuals MUST physically have a legal and valid US/Canadian government issued picture ID or a passport book. The form and ID will be checked by staff. The form is good for one (1) year.

BELLEVUE INDOOR RANGE SAFETY RULES:

  1. I swear and affirm that I am of good character and moral standing. I am legally entitled to possess a firearm. I have no criminal convictions and no pending criminal charges or indictments. I am not suffering from any forms of mental illness that would prohibit me from legally and safely using or possessing a firearm.

  2. I swear and affirm that I am not under the influence of any alcohol or controlled substance. I am not under any impairment that would prohibit me from legally and/or safely using or possessing a firearm. Bellevue Indoor Range will refuse service to any individual that may seem impaired.

  3. I will ALWAYS stay behind the firing line, which is the front of the shooting booth. No part of my body, including my arms and legs, will ever be forward of the firing line.

  4. I will fully enter a shooting booth BEFORE uncasing or unholstering a gun. The gun must be unloaded and fully cased/holstered before leaving the shooting booth. No gun is to ever be uncased or unholstered outside a shooting booth.

  5. I WILL NEVER WALK AROUND WITH FIREARM(S) IN HAND.

  6. I will ALWAYS set the gun down first before doing ANYTHING else. The gun must be on the table before looking at or speaking to ANYONE. I will be paying attention to the gun whenever it is in my hand.

  7. The muzzle of the gun will ALWAYS be pointed down-range into the bullet traps. No uncased/unholstered gun will EVER be pointed at ANYTHING and/or ANY DIRECTION but down-range into the bullet traps.

  8. I will ALWAYS keep my finger off the trigger until my sights are on-target and I have decided to fire. My finger will be off the trigger while manipulating the gun. This includes loading, unloading, or looking at the gun and/or target.

  9. I will only fire at paper targets hung from the carrier in my assigned lane. Cross-lane firing or shooting at anything other than my own paper target is strictly prohibited. Any direct, indirect, consequential, or incidental shooting of walls, floor, ceiling, lights, baffles, carriers, clips, or any other range property or equipment will result in removal from the range. The person will be barred from future use of Bellevue Indoor Range facilities and, if necessary, reported to the proper authorities for appropriate action.

  10. I will not practice drawing from the holster. DRAWING FROM THE HOLSTER IS PROHIBITED unless I am a member of Bellevue Indoor Range or an active law enforcement officer, and I have been approved by range staff.

  11. I will not kneel or sit while shooting, unless it has been approved by staff. NO KNEELING OR SITTING IS ALLOWED unless it has been approved by staff

  12. I will only have range accepted ammunition. All ammunition in the range must be brass cased and be partially or fully jacketed. STEEL AND ALUMINUM CASED AMMUNITION IS SPECIFICALLY PROHIBITED. Lead round nose (besides .22LR), wad cutter, or semi-wad cutter is specifically prohibited. Condensed lead or steel-core ammunition is specifically prohibited. Other restrictions may apply. Some rare exceptions may exist, but it is the responsibility of the individual to check with staff beforehand. ALL AMMUNITION IS SUBJECT TO INSPECTION.

  13. I will ONLY SHOOT RANGE AMMUNITION THROUGH RENTAL GUNS. Unused ammo may be taken but will not be refunded nor will it be accepted for use in rental guns on future visits. All ammo for rental guns must be purchased through Bellevue Indoor Range or Wade’s Eastside Guns at the time of the rental.

  14. I will only retrieve spent brass fired by myself. I will only retrieve brass within my shooting booth. I will not attempt to retrieve any brass forward of the firing line, outside of my booth, or fired by others. DO NOT EVER REACH FORWARD OF THE FIRING LINE. A cease fire will not be called to retrieve brass. Leave all other brass on the range.

  15. I will wear eye protection (minimum of 45% light transmission) and ear protection (minimum NRR of 28) prior to entering the range and at ALL TIMES while on the range. Sunglasses and eye protection with dark tints will NOT be allowed in the range. EYE/EAR PROTECTION MUST BE ON ALL AT TIMES while on the range.

  16. I will NOT take any pictures and/or videos involving anyone else, or their property, without prior consent. All pictures/videos must be done safely and in full accordance with Bellevue Indoor Range rules.

  17. NO food, beverage, or tobacco use allowed in the range.

  18. Any shooter under 18 years old must have an adult guardian within arm-distance at ALL TIME. ANY PERSON UNDER THE AGE OF 18 MUST BE ACCOMPANIED BY THEIR PARENT OR LEGAL GUARDIAN. THE MINOR MUST HAVE THE SAME LAST NAME AS THEIR PARENTS OR BRING AN AUTHORIZATION LETTER FROM THE PARENT OR LEGAL GUARDIAN WITH THE SAME LAST NAME OR A LEGAL DOCUMENT THAT PROVES THEY ARE THE PARENT OR LEGAL GUARDIAN OF THAT MINOR IN ORDER TO BE ABLE TO SHOOT AT THE RANGE ON THAT DAY.

  19. Any and all training and/or qualification is PROHIBITED without prior range approval and/or contract.

  20. I fully understand the universal firearm safety rules and will adhere to those rules at ALL TIME.

  21. Any violations, non-compliance, unsafe practice, or questionable conducts may result in the immediate removal of the individual, and all associated individuals, from the facility without any refunds or reimbursements.

UNIVERSAL FIREARM SAFETY RULES:

  1. Always treat all guns as loaded.

ASSUMPTION OF RISK, RELEASE OF ALL CLAIMS AND INDEMNITY AGREEMENT

   I understand that there are inherent and other risks involved in the use of firearms up to and including, bullet ricochets and/or “splash backs”. Splash backs are bullets that hit steel or other hard surfaces and fragment. The fragments fly back toward the firing line and may cause bodily harm if it strikes a shooter or observer. A range shooter can shoot themselves or others or be shot by another range shooter by accident. I freely assume those risks, including the risk of serious injury or death.

I also understand that shooting firearms will cause LEAD and NOISE exposure as both are emitted from shooting. Bullets and primers are made of lead, cleaning firearms or even handling firearms causes lead exposure. The sound percussions are also created by firearms and can cause irreversible hearing damage. Proper hearing protection is mandatory while on the range. Improper firearms handling can lead to burns, cuts, or minor and/or serious injuries.

I agree to RELEASE, HOLD HARMLESS AND INDEMNIFY the Bellevue Indoor Range / Wade’s Eastside Guns, the manufacturers and distributors of firearms, ammunition and accessories, and their owners, agents and employees from any and all liability for damage and injury to myself or any person or property resulting from any cause, including but not limited to negligence, improper selection, adjustment and use of firearms and ammunition.

I will accept for use, as is, any equipment rented or provided to me and accept full responsibility for its use. I will be financially responsible for replacement at full retail value, any equipment rented or provided under this agreement that is not returned to the Bellevue Indoor Range and will reimburse the Bellevue Indoor Range for any loss or damage, other than reasonable wear and tear. I will return all equipment rented or provided prior to departing the building.

I agree that it is my responsibility to know how to properly and safely use any equipment rented or provided to me. I will ask for any instruction or direction necessary prior to use. I understand the function of the equipment and will not misuse or abuse the equipment. If I rent a firearm from the Bellevue Indoor range I agree I must use their ammunition.

I am not legally restricted from possessing firearms.

I will not provide services such as firearms training in exchange for money to any person or persons on the premises of the Bellevue Indoor Range.

I am legally competent to sign this release or my parent or guardian has also read and signed this release. If I am less than 21 years of age, signature of a parent or legal guardian over 21 is required. Exceptions would be shooters 18 years old and renting or shooting rifles only.

I will immediately obey any safety directives or warnings given to me by any member of the Bellevue Indoor Range staff. I further agree that if I fail to immediately obey any directive I will remove myself and my belongings from the premises unless asked to remain pending the arrival of law enforcement personnel.

I recognize that Bellevue Indoor Range, INC. would never rent me any firearm(s) if it had any reason to suspect that these oaths or affirmations are untruthful in anyway, and if I violate any of these oaths or affirmations, I accept full and sole responsibilities for my actions, and I agree to allow Bellevue Indoor Range, INC. to use this document in any form whatsoever to demonstrate my deceit, dishonesty and misrepresentations.

I have read this agreement in its entirety and fully understand its contents. I am aware that this release of liability is a binding contract and I sign of my own free will. I also agree to present legal and valid identification.
