- I give my consent for this child/minor to travel on sightseeing tours through Western Canada and the USA operated by Uniglobe Specialty Travel Ltd. and its various DBA divisions including Discover Canada Tours, West Trek Tours, DC Student Adventures, Club ESL, and Destination Snow.

By checking this box, I hereby acknowledge and confirm that I have informed my child/the participant (name listed below) of the following:
Sightseeing tours and other activities including the optional activities associated therewith (collectively the "Tours") sold, purchased, booked, operated or administered by Uniglobe Specialty Travel Ltd. and its various DBA divisions including Discover Canada Tours, DC Student Adventures, Club ESL, West Trek Tours, and Destination Snow (collectively "Uniglobe") may be dangerous, exposing participants to many risks and hazards including bodily injury, property damage and death and that such injury, damage or loss may be inherent in the very nature of the Tours and result from human error and negligence on the part of the persons involved in preparing, organizing, staging, administering and operating the Tours;
Some of the aforesaid risks and hazards are foreseeable and may be caused by human error including negligence; They have carefully read this ACKNOWLEDGEMENT OF RISK FORM, fully understand same and are freely and voluntarily executing same; They are physically and mentally capable of participating in the Tour and they have no pre-existing conditions that would hinder their ability to participate in the Tour. By participating in this tour, they also agree to following the code of conduct for behaviour on tour and understand that failure to comply may result in removal from the tour at their expense Participants will: - Follow the instructions of staff and supervising adults, without delay or argument
- Take responsibility for their actions and do nothing to endanger themselves or others
- Report all accidents or damage to the group leader or a member of staff
- Show respect and act with courtesy towards peers and staff on tour, including the general public and suppliers
- Be helpful, co-operative and considerate to others, including the general public
Participants will not: - Smoke
- Consume alcohol if under the legal drinking age of the jurisdiction they are in
- Take illegal substances
- Act inappropriately in a way that will endanger themselves or others
- Use inappropriate language
- Leave the group and go off on their own
- Take part in any activity which has not been authorised by their parents or group leader
