WAIVER RELEASE OF LIABILITY AND HOLD HARMLESS
This is an important legal document, please read and understand it before you sign

I (OR the parents/guardian of), the particpant, Have requested an opportunity to participate in the “Discovery Light Sport (LSA) Airplane, Airplane or Helicopter Flight Program” OR the “LSA, Airplane or Helicopter Scenic Flight Tour Program” of Snohomish Flying Service, Inc. Our Light Sport Airplane, the Evektor was designed and manufactured under rigid, voluntary, industry consensus standards.

I fully understand flying is dangerous, the outcome is not assured and seen or unforeseen consequences may cause me/them to suffer injury, damage, loss or even death. In any event and regardless of circumstances or cause, by this agreement. I release SNOHOMISH FLYING SERVICE, INC. their stockholders, agents, assigns, and employees from any liability what-so-ever as a result of the flight and further agree to indemnify/hold them harmless to myself, my estate, or any third party which might suffer damage from this act (in any form or measure). I understand that this release is voluntary and that I am both waiving some important rights as well as taking on some legal responsibilities but unless I sign this form, I may not participate in the Flight Services as requested. Having weighed the risks versus the benefit, I agree to assume the risk and my signature indicated acceptance of this contract to hold harmless and release from liability. If I am executing this release on behalf of a minor, I do certify that I have the authority to do so, and that I am the legal/natural guardian of the minor involved, and have executed the release with the full knowledge, consent, and desire of the minor.
