NOTICE TO THE PET OWNER/GUARDIAN: PLEASE READ CAREFULLY. THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHT TO KAYLA STYGER (referred to in this agreement as “WHISTLER DOG NANNY ”), AND RELATED PARTIES. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY AND UNDERSTAND ITS EFFECT, POLICIES, PROCEDURES, PET RELEASE, WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNIFICATION AGREEMENTS.

WAIVER AND RELEASE OF LIABILITY

THIS AGREEMENT is entered into by and between Whistler Dog Nanny and the party who's name is typed below (“Dog Owner”):
By enrolling the below named and described dog(s), I represent that I am the legal owner of the named dog(s) and I assume all risks, dangers and responsibility for injuries to the named dog(s). Dog Owner understands and agrees that Owner is solely responsible for any harm to Dog Owner’s dog(s) while Dog Owner’s dog(s) is under the care of Whistler Dog Nanny. AGREED AND ACCEPTED -

Owner understands and agrees that during normal dog play, Owner’s dog may sustain injuries. All dog play is monitored by Whistler Dog Nanny to avoid injury, but scratches, punctures, torn ligaments, and other injuries may occur despite the best supervision.

In consideration for my pet (s) as a customer of WHISTLER DOG NANNY, by signing this document Owner/Guardian, make the following representations, certify the accuracy of all information provided to Whistler Dog Nanny at any time, and agree to all the following policies, procedures, terms and conditions stated below in this Whistler Dog Nanny Agreement, Policies, Procedures, Terms and Conditions.

The Pet Owner will provide all required leashes, harnesses, muzzles, jackets collars, cages, food, (or any other items deemed required for the pet's service). The Pet Owner agrees to the following terms and conditions:

The Pet Owner agrees to provide WHISTLER DOG NANNY with all necessary instructions for their pets requirements while under their care.

The Pet Owner will provide any medication or special needs instructions for each of the pets The Pet Owner will be considered liable for any damages or injuries caused by their pets as well as to any other pets while under the care of WHISTLER DOG NANNY. I am aware that there is a surcharge on major holiday weekends.

WHISTLER DOG NANNY will perform the duties listed by the Pet Owner to the best of {his/her} abilities, in a reliable and caring manner.

WHISTLER DOG NANNY will keep secure and confidential any keys, pass codes, access numbers and personal information, and return them at the end of the contracted service date

WHISTLER DOG NANNY will not be liable for any injuries to the dogs that are caused by animals or people outside of WHISTLER DOG NANNY control.

WHISTLER DOG NANNY will not be liable for any incident that occurs during transportation of the dogs to a trail, lake, dog park or vet clinic (if an emergency arises).

Pet Requirements 

I represent that my pet meets all of the following requirements:

  • Is current on his/her required vaccinations,
  • Has been in good health for the last 10 days
  • My pet is not aggressive towards other dogs or humans and is not toy protective (unless previously discussed and necessary precautionary methods have been taken.)

Owner further understands and agrees that neither Whistler Dog Nanny nor any of its employees, staff or volunteers, will be liable for any illness, injury, death, and /or escape of Dog Owner’s dog(s) provided that reasonable care and precautions are followed, and Dog Owner hereby releases all of them of any liability of any kind whatsoever arising from or as a result of Dog Owner’s dog(s) while in the care of Whistler Dog Nanny

Dog Owner further understands and agrees that any behavioural or physical health problems that develop with the Dog Owner’s dog(s) while with Whistler Dog Nanny will be handled and treated as deemed best by the employees, staff and volunteers of Whistler Dog Nanny in their sole discretion, and Dog Owner expressly agrees to assume full financial responsibility for any and all expenses arising or relating thereto. This includes aggressive or destructive behaviour.

Owner further understands and agrees that Dog Owner’s dog(s) are healthy and will at all times while in the care of Whistler Dog Nanny have their current vaccinations. Dog Owner affirms the dog(s) they bring to Whistler Dog Nanny do not have any condition that could potentially jeopardize the health of other dogs or people and has not had any potentially communicable condition within 30 days prior to service or care. Dog Owner further understands that even if Dog Owner(s) dog is vaccinated for Bordatella (Kennel Cough) there is a chance that the Owner(s) dog can still contract Kennel Cough. I agree that I will NOT hold Whistler Dog Nanny responsible if Dog Owner’s dog(s) contracts Kennel Cough.

Dog Owner further understands and agrees that it is the Owner(s) responsibility to leave an adequate supply of food and medications for Owner(s) dog during the entire time Dog Owner(s) dog is cared for by Whistler Dog Nanny. Should the food/medication supply need replacement, I authorize Whistler Dog Nanny to purchase replacement and Dog Owner will reimburse Whistler Dog Nanny for cost plus a $25.00 replacement fee (per occurrence) plus the actual food and medication costs.

Whistler Dog Nanny reserves the right to refuse service to any dog or to dismiss any dog that does not meet the health, temperament or other Whistler Dog Nanny standards. The determination shall be made at the sole discretion of Whistler Dog Nanny.

Dog Owner hereby certifies that Dog Owner has read and understands this Waiver and Release of Liability and the regulations set forth above.

Parent(s) or court-appointed legal guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms of this document, as set forth above.
