CONSENT (SIGNED BY PARENT OR APPOINTED GUARDIAN IF PARTICIPANT IS UNDER 18) RELEASE OF LIABILITY AND ASSUMPTION OF RISK


In consideration of being permitted to use the trampolines and related facilities within Freedog Urban Activity Centres, the undersigned acknowledges and agrees to the following:

OUR DUTY OF CARE
The rules of Freedog Bristol set out below are not intended to limit your enjoyment ofthe facilities.
They are part ofthe duty of care that we, as operators, owe to you,the customer, by law. As such they
are not negotiable and if you are not prepared to abide by them then the staff may politely ask you to
leave.


YOUR DUTY OF CARE
You also have a duty of care to act responsibly towards the other users of the centre. Statements of
‘Good Practice’ are posted around the centre adjacent to the relevant facilities.


PHOTOS
Freedog Bristol may use any photos taken of you in the centre on our website and social media. You
mustinform the Duty Manager every time you enter the building ifthis is not the case.


ALCOHOL AND DRUGS
Being under the influence of drugs or alcohol when using Freedog Bristol’s facilities is strictly forbidden. The acceptable level of alcohol in any participants system is zero. If the staff suspect or
know, a participant has had an alcoholic drink they may be refused participation or entry.


GENERAL

  • Running is not allowed anywhere in the centre (excluding the sprung floor).
  • Participants must report to reception on each visit before they use the centre.
  • Participants must exercise care, common sense and self-preservation at alltimes.
  • For the safety of all participants and others, if a staff member does not think a participant is capable of completing a move safely,they will communicate this to the participants and the participant must then stop.
  • Participants must report problems with the trampolines equipment or other participants behaviour to a member of staff as soon as possible.
  • Freedog branded socks must be worn on all activities at alltimes.
  • Due to our manufactures guidelines regarding the trampolines a maximum weightlimit of 140kg is enforced.
  • Freedog Bristol reserves the rightto refuse entry.
  • Participants must be aware ofthe other customers around them and how their actions will affect them.
  • Participants to the best oftheir knowledge, should not suffer from any medical condition or other reason which may preventthem from participating in any of the Activities or which would make it more likely thatthey would be involved in an incident which could resultin injury to themselves or others.
  • Do not distract people while they are using Freedog Bristol’s facilities.
  • Instructors/staff are on hand to attend to any First Aid requirements. If a participant has a minor injury, itis their responsibility to inform the instructor/staff member.
  • Each participantis responsible for his/her actions and those of his/her children on and around the centre and agrees to abide by all posted rules, policies and procedures in order to maintain the utmostlevel of safety;
  • Children under the age oftwelve must have an accompanying adult within the building.

Use Of Freedogs Trampoline Park:

  • Participants will abide by all policies and procedures regarding all trampoline park activities; participants will watch the safety video and read all safety-related signs before using the facilities at Freedog and satisfy  themselves thatthey have fully understood all instructions.
  • Participants must understand thattrampoline activities involve a certain level of risk of injury.
  • Participation in these activities is completely voluntary.
  • Children under the age oftwelve must have an accompanying adult within the building.
  • All Participants must understand and know their own limits. Participants must nottry anything they feel may be unsafe or dangerous.


Use of Freedogs Ninja course:

  • Participants will abide by all policies and procedures regarding the Ninja warrior course; participants will watch the safety video and read all safety-related signs before using the facility at Freedog Bristol and satisfy themselves thatthey have fully understood all instructions.
  • Participants must understand thatthe Ninja course involves a certain level of risk of injury. Participation in this activity is completely voluntary.
  • Participants under the age of 5 are not permitted to use the ninja warrior activity.
  • Only one person per activity per line.
  • All Participants must understand and know their own limits. Participants must not try anything they feel may be unsafe or dangerous.


Use of Freedogs Toddler Sessions:

  • Our toddler sessions are to provide fun and enjoymentfor children aged 5 and under AND under the height of 110cm.
  • Participants must receive a safety brieng every time before taking partin the session.
  • Accompanying adults must keep their bouncing to a minimum and refrain from any ips, tricks or summersaults anywhere during these sessions,this includes bouncing offthe sidewalls and extreme bouncing into the foam pit.
  • Accompanying adults must be in direct supervision at alltimes.
  • Adults must make sure their bouncing is on their own trampoline and be aware of other toddlers and adults around them as they may collide.
  • Participants must respect and not damage the toys provided.
  • Accompanying adults agree thattheir child/toddler is fully able to participate in the session and has no injuries/illness or medical conditions that may be adversely affected by participation.
  • Due to the height restraints,the ninja warrior course is notto be used in this session.
  • The skills area may be in use for this session, however,toddlers and accompanying adults are not allowed to use the bars located inside ofthe skills area.


Use of Freedogs Skills Area:

 

  • Participants will abide by all policies and procedures regarding the skills area.
  • Participants need to fully understand thatthe skills area involves a certain level of risk of injury.
  • Participation in these activities is completely voluntary.
  • All participants must understand and know their own limits, participants must nottry anything they feel may be unsafe or dangerous.
  • Unsupervised open sessions on the skills area are justthat. Participants in unsupervised sessions must take full responsibility for their own safety and must not perform skills and tricks that are beyond their own capability.
  • In an unsupervised session, you must always familiarise yourself with the rules and abide by them at all times.
  • Supervised sessions are monitored by a Freedog member of staff. Participants must abide by the rules and their instructions. Although the session is monitored itis up to the participant whether
  • or notthey can complete a manoeuvre safely.


Use of Freedog’s Soft Play:

 

  • Whilst children are using the soft play area their accompanying adults must remain on-site at all times, and are solely responsible for their child(ren)’s safety.
  • This Session is an unsupervised session. Itis up to the accompanying adultto ensure the safety of the participant.
  • Only participants 10 and under AND under the height of 135cm are allowed to use the main soft play area.
  • Accompanying adults must read the rules located outside the soft play area before their child takes partin the activity. Itis the accompanying adult’s responsibility to ensure their child(ren) abide by these rules.
  • Food and drink must not be taken into the toddler area or on to any of the play equipment without exception.
  • Participants must respect and not damage the toys provided.
     

Legal declaration :

 

  • I hereby assume all ofthe risks of participation in the trampoline park activities and will hold the owner/operator and its employees, agents, ofcers,trustees and afliates harmless from any and all liability, actions, demands, damages, expenses, costs, claims and causes of action of any possible nature in respect of injury, death, loss or damage to myself, child or property however caused as a result of or in any way relating to my activities on and around the trampoline;
  • I further agree to indemnify and hold harmless the owner/operator, its employees, agents, officers, trustees and affiliates from and against any and all liability incurred as a result of or in any manner related to my participation in trampoline activities;
  • If despite the signing ofthis waiver, a lawsuitis brought againstthe owner/operator, its employees, agents, ofcers,trustees or afliates in relation to participation in the trampoline activities, I agree to pay for any and all court costs and solicitors fees incurred as a result of such litigation;
  • I agree that if any provision of this release is found to be unenforceable or invalid in any way, the remaining provisions will remain in force and effect;
