The below policies are required to receive a service with Vera M Styling/ DBA Vera Salon and its stylists. Booking an appointment at Vera Salon is acceptance of these policies. By signing this agreement, the client agrees to the below policies for all current and future services the client receives at Vera Salon. CANCELLATION POLICY: Extension Application Appointments: A 2 or more week's notice is required to move or cancel your appointment in order to transfer the non-refundable retainer to a new day as we reserve 4-12 hours for extension applications. If you move or cancel your appointment within 2 weeks of your original appointment date you will forfeit your non-refundable retainer and will be required to pay a new retainer to make a new reservation. Color, Treatment & Maintenance: Appointments cancelled or rescheduled within 48 HOURS will be charged 50% of the scheduled appointment on your card on file or will forfeit their appointment retainer. No-Show/ no calls will be charged the full appointment fee and will be required to pre-pay for all future appointments. Guests, children (unless receiving a service), and pets are not allowed in the salon. Your stylist may not be able to perform your service in full or at all if this policy is broken, and will result in loss of your deposit to account for the stylist’s time for that day. Retainer: Extension Application appointments require a non-refundable deposit/ retainer to book an appointment. Current clients (those currently wearing extensions applied at Vera Salon) retainer requirements are as follows for keratin and invisible cold extensions: $500 for side fill applications, $1000 for partial and full applications, $1500 for corrective and extra full/ long applications. A 50% retainer is required for new and non-current clients and tape-in clients to reserve an extensions appointment. If more hair and time is required to complete your look than initially quoted, the required additional cost will be removed from your retainer. Card on File: A current credit card is required for storing on file to book a service at Vera Salon. In the event that we do not receive a 2 or more week's notice to move or cancel an extension application appointment or a 48 hour's or more notice for color, extension maintenance or treatment appointments, the card on file will be charged 50% of the booked appointments total for color, treatment and maintenance appointments, and additionally will forfeit the retainer for extension application appointments. In the event that a retainer has not been paid, the card on file will be charged for the required retainer. I have read and agree to the cancellation policy, required non-refundable retainer to reserve an appointment date, and credit card policy above. COLOR APPOINTMENTS: Color Corrections: More time is required for a corrective color. Each color correction is unique. More than one visit will likely be required to achieve the desired result. Specific products and treatments will be prescribed and required to be followed in order to get the desired results. Reconstructive procedures may be required. Charges will be required at the end of each salon visit even if one more visit is required. Results are not guaranteed as hair color and artistry are subjective and hair has its limitations. Color and other salon chemical treatments come with the risk of damaging your hair and or scalp. If you hair requires more time and color to achieve a color to blend with your extensions for the completion of your desired end goal, you will be charged for the additional time needed to create the look in additional to the original booked appointment. You will be charged for the service your stylist needs to create to achieve your final color goal. Your cost will be reflective of the time needed to achieve the goal. Vera Salon / Vera M Styling will not be held liable to any hair or skin damage as a result of your chemical service. Signing this form indicates that you read and understand that stylist will proceed with the color correction service and every effort will be applied to reach your goal. If the goal is not reached you will still be responsible for the payment of labor and product. This releases the stylist and owner free from any liability claim arising from the application of hair coloring products. Color corrections are priced hourly per your stylist's hourly charge. EXTENSIONS APPOINTMENTS: Consultation: A consultation is required to book an extensions application appointment. It is complementary to current clients or when booking extensions the day of the consult. Otherwise, a consultation fee is required. A current client is one currently wearing extensions applied at Vera M Styling/ Vera Salon. Once your stylist has chosen your hair extension colors, length and amount of extension hair at the consultation, changes cannot me made. Please book a consultation 2 or more weeks before your extensions application appointment to allow for enough time to order/ customize / prep your hair for you. Extension Application Appointment: Extension Application appointments include hair extensions, application, cut, and styling. Color, Treatments, Tape-In Maintenance, and Removals are separate services. Your retainer and contract are for the services and time requested at the time of signing of the contract only. Services may not be taken away, but may be added on at the discretion of your stylist. Your requested extension colors, length, and amount may not be changed. If we have extra hair available, additional hair may be added for an additional charge to the original quoted price. Additional charges will be added on and your final total balance will change if your stylist needs to use more hair & time to complete your desired final look. Vera M Styling / Vera Salon's Stylists will make a good faith effort to achieve the client’s desired results. However, hair design work is a subjective art form, and the client acknowledges that the Stylist cannot and will not guarantee any particular results, including the client’s desired results for both color and extensions. Special Circumstances: Fine scalp hair, brittle, and highly bleached hair may result in hair loss. Should you decide to change your hair color after your retainer is paid, you will be required to pay a new deposit with a new consultation. Chlorine, sunscreen, and self-tanner will discolor the hair. We cannot guarantee the quality of the hair or the duration of the extensions staying in good shape, as we cannot monitor it once you leave the salon. Should the hair become damaged after your installation due to improper maintenance, you will be asked to buy new hair at full cost based on your stylist’s discretion. Certain medications cause hair loss. Ozempic, Semiglutide, Wegovy, Dexitrose, and similar medications may cause hair loss. You must inform your stylist if you are on or planning to be on these medications. If you are on a medication that affects your hair, it may weaken the natural hair and cause breakage from hair extensions. ExtensionsMaintenance: Expect to lose 20-30% or more of your extensions during the life between the application and recommended removal date. To best care for your hair, only use products recommended by your stylist. Keratin Extensions: must be brushed twice a day over your bonds following the proper brushing technique and cleansing your stylist has taught you. Individual Cold Extensions: do not brush over your bonds harshly, be very gentle and comb/ brush lightly over them. It is recommended instead to separate gently with your fingers. Be careful not to pull too hard. Do not use any oil or bonding products such as K-18 or Olaplex products near the bonds. They will all cause tangling and bond break down. Brush below the bond, ends up. Tape-Ins: Brush below the bond, ends up and not over the bond. Only use products recommended and sold to you by your stylist. Other products may cause bond breakdown which will case slippage and/or tangling, and possible damage to the hair. If extensions are matted and tangled, your stylist will remove them and at full cost. Sweat, humidity, hot yoga, improper cleansing practices, excess scalp oil, some medications, and using products not recommended by your stylist may result in extension loss and slippage. Avoid products with oils and direct high heat on the bonds, as this will cause slippage. Please ask your stylist for a list of medications that may affect hair extension results. I have read, and agree to follow the maintenance requirements outlined for hair extensions. I agree not to use products with alcohol or oils near or on my bonds. I understand that these will cause slippage. I understand that if I will need to remove and or re-apply my extensions sooner than the recommended average removal date if I don’t follow the recommended maintenance requirements, use products that I did not purchase from Vera Salon, and participate in the activities outlined above that cause extension bond breakdown and slippage. I agree to tell my stylist if I start weight loss medications, as these may create hair loss and agree to not hold my stylist or Vera Salon liable should I experience hair loss or hair damage on these medications. Removal: Extension Removals are $150/hr and are a separate service to keratin and invisible cold extension applications, invisible cold extension re-applications or tape-in move-ups. Removals should only be done by your stylist, or a Great Lengths certified professional. Do not attempt to remove your own extensions as this may cause damage. If you have excessive matting or tangling, your stylist may choose to not apply extensions on you. It is the client’s responsibility to book their removal at the recommended removal schedule below. Going past the removal schedule may result in damage to the natural hair and longer removal time. If you have tangling, matting, or hardened bonds due to improper home maintenance, your removal make take longer than the booked amount of time and the services following your removal will may need to be booked on another day. Removals should take on average 1 hr for a partial, 2 hrs for a full, and 3 hrs for a corrective set of individual extensions; and 1 hr for a full set of tape in extensions. Please plan ahead for removing your extensions and, if desired, booking a new application by following the recommended maximum length of time until removal for your extensions schedule below: Keratin & ICE Extensions: Fine Density: 2-3 months Medium Density: 3-4 months Thick Density: 4-5 months Tape-In Extensions: Fine Density: 4-5 weeks Medium- Thick Density: 5-6 weeks Allergy Test: The client waives the right to an allergy test unless the client asks for one.
