Wellness Journey (Limited Membership) Terms and Benefits

  1. Wellness Journey Membership - Treatments or Services:
    1. Each month you recieve two (2) 90 minute Signature Wellness Sessions with Gabi
      1. These can be used in office or remotely if needed/preferred
  2. Additional Discounts:
    1. 20% discount on any other (full-priced) Wellness Services
      1. Mind-Body-Energy Healing Session
      2. Hypnotherapy
      3. Reiki & Relaxation
      4. Reiki-Chakra Clearing & Balancing
      5. Remote Wellness Sessions
    2. 20% discount on Classes or Group Sessions with Gabi
      1. If available during the "Journey" period
  3. Membership Assignment:
    1. This may be shared with ONE other individual
    2. You cannot 'swap out' shared person during the 4 month period
  4. Rollover Period:
    1. Any unused monthly session(s) will remain available for 90 days, after which point it will expire.
    2. Upon termination of your 4 month membership, accrued member services expire and are non-refundable after 90 days
  5. Membership freeze option:
    1. Membership can be frozen for a fee of $20.00 per month
    2. Frozen months do not count towards the 4-month period
    3. Regardless of the length of the Freeze, the length of the Wellness Journey Membership will include 4 'active' months
    4. No services accrue and discounts are not usable during Freeze period
    5. Freeze must be requested in writing; please allow up to 72 hours for processing
    6. A Freeze is entered into the system for a specific amount of time, you can unfreeze prior to the initially requested period (request in writing, please allow up to 72 hours for processing)
  6. Membership Payment:
    1. Wellness Journey Membership fees are non-refundable.
    2. The membership fee will be charged on the same date monthly for 4 months. 
    3. Monthly Sessions will be available immediately after the charge clears. 
    4. SpaFinder, SpaWeek and gift certificates/cards are not accepted as forms of payment.
    5. We are happy to attempt to use HSA cards, but we have no control over your companies policies nor can make any guarantees of success/acceptability
  7. Additional Terms:
    1. All included services will  be provided by Gabi Hawk, CRMT, CCHt. 
      1. In the event that additional wellness practitioners are added to the practice, their services will not be included.
    2. This signed membership agreement must be on file.
    3. You may purchase this membership concurrently with the Sonoran Serenity Membership
      1. Serenity Members are eligible for a discount on their Wellness Membership

    4. Members must be at least 18 years old.
    5. The Wellness Journey (Limited Membership) does not automatically renew at the end of the term.
  8. Cancellation Policy:
    1. The standard appointment cancellation policy applies to all member appointments.
    2. Appointments may be canceled without penalty up to 12 hours before the scheduled appointment time.
    3. After 12 hours and before 2 hours of the scheduled appointment time client will be assessed a late cancel fee of 50% of the full price of the treatment or service.
    4. If an appointment is canceled within 2 hours of the appointment time or the client no-shows the appointment a late cancel/no-show fee of 100% off the full price of the treatment or service will be assessed. 
    5. Monthly benefits or accrued benefits may not be preferentially used as form of payment in the case of a late cancel or no-show.
    6. In the event that payment does not clear in a reasonable period of time, the Owner or a Manager may at their discretion use a service credit to 'pay for' the missed service

By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it.
