I, the undersigned, understand and will comply with all instructions regarding my sunless airbrush tan. I hereby authorize and direct employees or agents of LUXE Custom Airbrush Tanning to perform the tanning procedure. I have provided LUXE Custom Airbrush Tanning with the information required to perform such service. I acknowledge that results of sunless tanning can vary due to things such as personal skin care product remaining on my skin, medications, hormones and DHA resistance. I understand that no guarantee of results is offered or implied. I hereby relieve LUXE Custom Airbrush Tanning, as well as the technician, and hold them harmless from any liability involved in the tanning process. LUXE Custom Airbrush Tanning and their agents or employees are not liable for any injury to person or property or the loss or theft of any personal property. 

I affirm that I am at least 18 years of age and in good health (parental signature required of under the age of 18.) If I have a history of asthma, a respiratory condition, an allergy that may be aggravated by receiving a sunless tan, or am pregnant, I understand I need to consult a physician before receiving the sunless tan. I also understand that if I am currently on any medications, I am aware that the dihydroxyacetone (DHA) in solutions can interact with medications and result in a lighter or darker than usual tan. 

I understand that the FDA does not currently regulate spray tanning but they do suggest that anyone using DHA spray tanning services avoid inhaling or ingesting (DHA), the active ingredient in all sunless tanning solutions. It is recommended that I protect mucous membranes by using protective eyewear, nose filters, lip balm, and protective undergarments. LUXE Custom Airbrush Tanning will provide all these protective measures if I choose to use them. 

I also understand the tanning solutions used by LUXE Custom Airbrush Tanning contain cosmetic bronzers which could potentially rub off on clothing, furniture and car seats. It is best to wear dark colored swimsuits or undergarments if I choose to wear something while receiving my spray tan and dark loose clothing after my session. It's also advisable to place a towel/blanket down on car seats and furniture while wearing a spray tan. LUXE Custom Airbrush Tanning will not be held liable for any damage as a result of bronzer transferring onto items.  

All ingredients used in the tanning solutions used by LUXE are approved for cosmetic use. If I have any allergies to such things as nuts, dyes, cosmetic ingredients, or latex (gloves), I'll inform LUXE as soon as possible and request an ingredient list prior to my scheduled appointment. 

Each person is unique and that includes the way a person's skin takes to solution. I understand some clients will tan lighter and others will tan darker. LUXE Custom Airbrush Tanning makes no guarantees as to how dark each client will get. On the same note, each person's skin exfoliates at a different rate so the length of time a spray tan will last will vary with each person. Tans can last anywhere from 5-10 days with the average tan lasting about a week. LUXE Custom Airbrush Tanning sells products to help maximize tanning results. 

I understand this waiver is onging and it is my responsibiity to inform LUXE Custom Airbrush Tanning if any information changes.

The NIGHT BEFORE my sunless tanning appointment, I will make sure all waxing, shaving, manicures and pedicures are complete. I'll also make sure to shower and and then thoroughly exfoliate my skin with an exfoliating mitt or gloves (no loofahs) paying special attention to neck, underarms, shins and dry skin areas. I will not apply deodorant, make-up, perfumes, lotions or oils to my skin after exfoliating.
If I need to rinse the day of my tanning appointment, I'll do so at least 4 hours prior to my appointment with a wash cloth and warm water only. (Body wash can leave a residue which can cause a barrier on the skin. Hot water will irritate skin and change the ph which can cause the tanning solution not to take well.)
I understand dark, loose fitting clothing along with loose fitting flip flops are ideal to slip on after getting sprayed. If I choose to wear shorts, skirts or dresses, I know I need to be cautious of skin on skin (crossing legs, crossing ankles, resting arms on torso) because the tan can transfer and cause one area to be darker and the other area lighter Tight clothing such as sports bras, leggings, jeans, socks and running shoes can also cause issues in how the tan develops.
I understand sunless tanning solutions do not contain sunscreen so I will need to apply a LOTION sunscreen to protect my skin from sun damage.
I understand the after care products I use while wearing a spray tan play an important role in how long the tan will last and how well it will fade. A clear, liquid body wash without sulfates is best and moisturizers that do not contain mineral oil or petroleum/petrolatum work best with spray tans.
I understand I need to hold off on using acne, anti-aging or exfoliating products while wearing a spray tan. These items will drastically shorten the life of my tan.
I understand exercising and participating in hot yoga can shorten the life of my spray tan. Areas that tend to have friction (sports bra, leggings, socks/shoes) may fade quicker.
I understand if I have large breasts and want the tan to be seamless underneath, I will need to wear a bathing suit top, bra or bandeau while getting sprayed. If I choose not to wear a top, I understand my body temperature, the heat in Florida and having skin on skin rubbing may cause the tan under my breasts to be less than perfect. LUXE will do everything possible to make that area seamless by spraying, drying and applying powder to set the tan.
I understand if I've had a sunburn in the last 90 days, the skin may still be damaged. It is recommended not to apply spray tanning solution in that area but if you choose to have a spray tan applied, spray tanning solution may cause that skin to peel. Peeling may also happen for several weeks after skin treatments like lasers, chemical peels, skin renewing facials, etc.
I understand if I get sprayed while on my period, my tan may be lighter than normal and more likely to fade faster. DHA, the main ingredient in tanning solutions, is formulated to work within a certain pH range on your skin. Hormones can alter the pH on your skin which can result in a lighter tan.
