1.     We promote responsible tanning and therefore tanning is permitted ONCE EVERY 24 HOURS. To avoid overexposure, we recommend that you do not tan out doors on days you are tanning indoors, and that you do not tan if you currently have a sunburn.

 

2.     WEAR PROTECTIVE EYEWEAR. You are required to wear protective eyewear while tanning in our facility. Failure to wear protective eyewear may result in severe burns or long-term injury to the eyes. We recommend that you remove contact lenses before tanning.

 

3.     Your session times are determined according to your skin type, tanning history as well as the condition of our equipment. We understand you may have been tanning elsewhere, but we know our equipment best. Please follow our advice. Failure to follow said advice may result in burning for which we cannot be held responsible.

 

4.     Mineral oil, baby oil, outdoor tanning oils and lotions, etc. are harmful to the tanning bed acrylics and are NOT permitted at Sun Haven. Please do not use any lotion that has not been approved by one of our staff members.

 

5.     A MAXIMUM of ONE (1) person is permitted in a room during the tanning session. Sun Haven does not permit spouses, friends, etc. in the room while a session is ongoing. This includes children and persons under the age of 18. If you choose to bring your child into the room, we cannot be held responsible for any injury caused by any way.

 

6.     PLEASE FOLLOW ALL INSTRUCTIONS regarding the operation of all tanning equipment. The proper procedure to follow in the tanning room has been clearly explained by the attendant. Please feel free to ask any questions or to voice any concerns that you might have before starting to tan.

Precautions are necessary for safe tanning. I have read the contents of this Client Release and Informed Consent form carefully and state that I am not aware of any medical condition or other reason that would prohibit me from tanning. I agree that I will comply with all instruction regarding use of tanning equipment and procedures, understand the risks involved, and use these services at my own risk. Sun Haven owners, employees, and agents are also not responsible for the loss of theft of any personal property. Each person is responsible for safeguarding their own property. I hereby agree that Sun Haven owners, employees, and agents are not liable for any injury to person or property caused by any way due to the use of this facility and/or services.

 

CORONAVIRUS (COVID-19) LIABILITY RELEASE WAIVER

Due to the outbreak of the novel Coronavirus (COVID-19), Sun Haven Tanning Inc. has put in place preventative measures to protect our clients, our community and our staff. Sun Haven Tanning Inc. will be following the Center of Disease-Control (CDC) recommendations with regard to social distancing practices and sanitation. Please read the following form carefully.

 

Symptoms of COVID-19 include:

  • Fever
  • Fatique
  • Dry Cough
  • Difficulty Breathing

I am not currently experiencing COVID 19 symptoms nor have I had COVID-19 symptoms in the last 14 days.

I have not travelled outside of Canada or in an area under a travel health advisory in the last 14 days.

I have not provided care or had close contact with any person with COVID-19 or with any person reasonably suspected of having COVID-19 or with any person who travelled outside of Canada in the last 14 days or with any person who travelled in an area under a travel health advisory in the last 14 days.

I have not been advised by the Government of Canada or Ontario Public Health or my doctor or the Ontario Ministry of Health website to self-isolate due to possible exposure to COVID-19.

I am fully aware of the risks and hazards with respect to COVID-19 inherent in my attendance at the Premises.

 

I hereby agree to release Sun Haven Tanning Inc. from any and all liability for the unintentional exposure or harm due to the Coronavirus(COVID-19).
