WAVIER & RELEASE OF LIABILITY FORM PLEASE READ CAREFULLY BEFORE SIGNING
 
In consideration of Grand Junction Stand Up Paddle/ Grand Junction Adventures LLC furnishing services and/ or equipment to enable me to participate in any of the following activities associated with stand-up paddle, rock climbing, mountain biking and hiking I agree as follow:
 
I fully understand and acknowledge that:
 
(A) Outdoor recreational activities have inherent risk, dangers and hazards and such exists in my use of stand-up paddle, mountain bike and rock climbing equipment and my participation in this activity;
 
(B) My participation in such activity and/ or use of such equipment may result in injury or illness including, but not limited to bodily injury, disease, strains, fracture, partial and/or total paralysis, death or other ailments that could cause serious disability;
 
(C) These risks and dangers may be caused by the negligence of the owners, employees, officers or agents of Grand Junction Stand Up Paddle/ Grand Junction Adventures LLC, the negligence of the participants, the negligence of others, accidents, breaches of contract, the forces of nature or other causes. Risks and dangers may arise from foreseeable causes including, but not limited to, guide decision making, including that a guide may misjudge terrain, weather, river or lake route location, and water level, risks of falling off or drowning while stand up paddle boarding, boating, kayaking and surfing, risk of falling rocks while climbing, triping on rocks while hiking, risk falling off mountain bike while biking on desert trails and such other risks, hazards and dangers that are integral to recreational activities that take place in a wilderness, rivers outdoor or recreational environment;
 
(D) By my participation in this activity and/or use of equipment, I hereby assume all risks, dangers, all responsibility for any losses and/or damages, whether caused in whole or in part by the negligence or other conduct of the owners, agents, officers, or employees of Grand Junction Stand Up Paddle/ Grand Junction Adventures LLC or by any other person. I hereby assume all finacial cost of lost and damages of equipment I am responsible with rental. In addition, I hereby grant permission to Grand Junction Stand Up Paddle/ Grand Junction Adventures LLC to make use for promotion or other purposes, photographic records without recourse or compensation to me; 
 
I, on behalf of myself, my personal representatives and my heirs or as parent or legal guardian of the undersigned participant hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Grand Junction Stand Up Paddle/ Grand Junction Adventures LLC, the State of Colorado, the United States of America, Bureau of Land Management and the City of Grand Junction, agents, officers and employees from any and all claims, actions or losses for bodily injury, property, damage, wrongful death, loss of services or otherwise which arise out of my use of stand up paddle board, kayak, boating, surfing, climbing, moutain biking and yoga equipment or my participation in lake or whitewater water craft activities, hiking & mountain biking desert trail activities and rock climbing activities .  I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have presently or in the future for the negligent acts or other conduct by the owners, agents, officers or employees of Grand Junction Stand UP Paddle/ Grand Junction Adventures LLC, the State of Colorado, the United States of America, Bureau of Land Management and City of Grand junction.
 
I HAVE READ THE ABOVE WAIVER AND RELEASE AND BY SIGNING IT, AGREE IT IS MY INTENTION TO EXEMPT AND RELIEVE GRAND JUNCTION STAND UP PADDLE/ GRAND JUNCTION ADVENTURES LLC, THE STATE OF COLORADO, THE UNITED STATES OF AMERICA, THE BUEAU OF LAND MANAGEMENT AND HIGHLINE LAKE STATE PARKS AND WILDLIFE FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE.
