Release and Consent Agreement

The undersigned student and/or parent or legal guardian of a student of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., by signing this contract, expressly acknowledges that this contract contains release and other risk-shifting provisions which may be operated to shift risk from The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., to the undersigned student and/or parent or legal guardian of a student of The Gravity Forge Gymnastics and

Ninja Warrior Training Center Inc., and the student and/or parent of legal guardian of a student of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., expressly accepts the responsibilities and duties resulting from such provisions. The individual(s) signing the agreement admit(s) reading and understanding the terms contained in this agreement. I (we) the undersigned student and/or parent or legal guardian of a student of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., for an in consideration of enrollment and/or the enrollment of the student, my child or a student for whom I have been granted legal custody herby voluntarily and knowingly execute this release with the express intention of effecting the extinguishments of and complete release from any and all claims, actions, demands or rights to monetary judgments whatsoever arising from any and all injury or physical harm which may occur to the student, including specifically those that may arise out of, or be occasioned by, directly or indirectly, any negligent act(s) or omission(s) of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., its officers, agents, employees or servants during the students attendance at and participation in any activities associated with The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., both on and off The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., premises. The undersigned expressly agrees that this release and waiver is intended to be as broad and inclusive as permitted by the laws of the Commonwealth of Pennsylvania, and that if any portion of this release and waiver is held to be invalid, it is agreed that the balance shall; notwithstanding, continue to full legal force and effect. This release contains the entire agreement between the parities and the terms of this release are contractual and not merely a recital.

Medical Consent and Release

I, the student and/or the undersigned parent or legal guardian of the above named student do hereby expressly grant authority to the staff of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., to render a judgment concerning medical assistance in the event of an accident, injury or illness during my absence and execute this consent and release provision with the express intention of effecting the extinguishments of and complete release from any and all claims, actions, demands or rights to monetary judgments whatsoever arising from any and all injury or physical harm which may arise from the rendering of such judgments, including, specifically those that may arise out of or be occasioned by, directly or indirectly, any negligent act(s) or omission(s) of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., its officers, agents, employees or servants involved in the rendering of such judgments. Furthermore, in the case of an emergency I consent and expressly grant the staff of The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., the authority to obtain medical assistance and treatment as they deem necessary. I understand that The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., its officers, agents, employees or servants shall not be responsible for any medical expenses incurred on behalf of the above named student, and that I am responsible for all payment of medical expenses so incurred.

I give permission and consent for a licensed doctor or physician to administer the necessary aid to my child or legal ward should he/she become injured or sick while in attendance at or while participating in any activity associated with The Gravity Forge Gymnastics and Ninja Warrior Training Center Inc., and to do so without having to wait until I (we) are contacted.

COVID 19

I am aware that my actions as a parent/guardian will also impact the safety of all others at The Gravity Forge Gymnastics and Ninja Warrior Training Center. We are counting on our parents to help us meet the physical-distancing and personal hygiene status recommended by the CDC guideline and the state of PA. I will do my part to help make The Gravity Forge Gymnastics and Ninja Warrior Training Center safe by being respectful of the gym social distancing policies, insisting that my child wash/sanitize their hands before entering the gym and arriving home and by supporting the instructors/coaches and The Gravity Forge Gymnastics and Ninja Warrior Training Center staff as they work hard to prove a safe place and develop happy, healthy and confident kids.

Assisting and Spotting

I am aware that gymnastics, tumbling, and warrior activities is an individual spot, where there will be times when incidental contact may occur. The Gravity Forge Gymnastics and Ninja Warrior Training Center programs are operating in a social and physical distancing environment but even with the best efforts and intentions there will be times where the children and instructors/coaches will breach the prescribed 6 ft social distancing recommendations. In additions, our instructing and coaching will need to physically assist and spot when the circumstances require it. Spotting and assisting our students and athletes is often necessary in order to teach skills safely and to help the students and athletes to perform skills correctly and to prevent injury. I understand and agree that spotting will be part of the safety and learning process at The Gravity Forge Gymnastics and Ninja Warrior Training Center. I agree to permit my child’s instructor/coach to physically assist and/or spot my child when needed. Direct assistance for safety will be provided in the event of a possible injury/illness.

Media and Promotional Material

The Gravity Forge Gymnastics and Ninja Warrior Training Center, "The Gravity Forge" retains the right to use any photographs, videotapes, motion picture recordings, or any other record of class programming, event, or stay at The Gravity Forge for The Gravity Forge website, publicity, promotion, advertising, or any legitimate commercial purpose associated with The Gravity Forge and its businesses, in the sole discretion of The Gravity Forge. Use of a child's photo (without name) will only be used in compliance with COPPA and other applicable laws.
