Waiver of Liability Relating to COVID-19

The novel COVID-19, have been declared a worldwide pandemic by the World Health Organization and is reported to be extremely contagious. 

The Dancer Project Conservatory LLC AND The Dancer Project Inc. have taken extreme preventative measures to help stop any spread of any virus or disease but cannot in absolution prevent you [or your child(ren)] from becoming exposed to, contracting, or spreading COVID-19 while utilizing The Dancer Project Conservatory LLC or The Dancer Project Inc’s services or premises. If you choose to utilize The Dancer Project Conservatory LLC and The Dancer Project Inc.’s services and/or enter onto The Dancer Project Conservatory LLC and The Dancer Project Inc.’s premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19 in or around the immediate studio facilities.

I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against The Dancer Project Conservatory LLC and The Dancer Project Inc. and its owners, officers, directors, managers, officials, trustees, agents, employees, volunteers or other representatives from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
