Liability Release: To the best of my knowledge, my pet is current with vaccinations and my pet has no underlying medical conditions that I am aware of. I understand that if my pet has a medical condition that I cannot, or choose to not, vaccinate, or prefer a "no vaccine approach", there are risks of illness or disease even with the constant santization at this grooming facility and Compassionate Groom and Spa cannot be held liable. I understand that Compassionate Groom and Spa does not require pets over the age of 1 year of age to show proof of vaccination. Puppies must be up to date with age appropriate vaccines at the time of visit.
If my pet does have any health problems (especially heart conditions, seizures, and cancer), I have disclosed this information with the groomer and understand the risks associated with these conditions in combination with the possible stress of grooming. Compassionate Groom and Spa is not responsible for any accidental death of a pet that may have been caused by a pre-existing condition, natural disaster, or old age. Compassionate Groom and Spa is not responsible for injury to themselves due to their behavior (ie: very aggressive,feaful,etc) during grooming. Compassionate Groom and Spa is not responsible for clipper burn and/or minor nicks resulting from the grooming of matted or neglected fur coats, or for irritation caused by grooming pets that posses mild to severe skin allergies, hot spots, or dermatitis. Flea Policy: I understand that if fleas are noted during the grooming process my pet (dogs and cats) will be given a single dose of a size appropriate Capstar, at owner's expense, by mouth which is an OTC non-prescription product that is used to safely kill fleas on dogs and cats within 30 minutes. Compassionate Groom and Spa will contact me if they find live fleas prior to the administration of Capstar. I also understand that if my groomer is unable to get in contact with me within a reasonable time frame to still complete the groom, my pet will be administered a dose of Capstar and the groom will continue. I understand that Compassionate Groom and Spa is not responsible for any side effects that are possible with the administration of this product. This is to ensure the safety of all the pets that reside temporarily at the location of Compassionate Groom and Spa. I understand that if I do not want this product given at the time of fleas being noted, that I will be asked to pick up my pet immediately and grooming will not be completed and I will be charged a non-compliance fee. Cancellation, No Show, and Late Fee Policy: Clients who do not get in contact with at least a 48 hours notice of cancellation, or do not show up at all, will be required to pay a fee for the missed serviced.
By signing this form and initialing this section, you agree to any and all late fees, late cancellation fees, and no show fees *Less than 48 hours notice of cancellation will incur a 50% late cancellation fee of the groom price and if applicable, Fit in Fee. *Less than 24 hours notice of cancellation will be required to pay the full cost (100%) of the groom price and if applicable, Fit in Fee.

If you are unable to obtain your pet for their appointment, unfortunately a no show fee would still apply. If you booked multiple pets and you bring less than what was booked, a no-show fee would still apply. If you had a "Fit in" appointment with your groomer and you no show or cancel late, the fit in fee, in full, and the no show/late cancellation fee will be charged to the card on file.
This fee is dependent on the service and time frame of the appointment. This fee will be charged to the card on file. If we do not have your card on file , or your card is declined, we will contact you to update the information. If the fee is not paid when due, the balance will incur an additional 10% late fee per day for 90 days before being sent to collections. This fee will need to be paid in order to schedule another appointment and avoid being sent to collections. After 3 no shows/late cancellations, your next appointment would need to be paid in full at the time of booking. If you no show or cancel less than 24 hours before this appointment, the amount that was pre-paid is nonrefundable and nontransferable.
If you are 10 or more minutes late to your appointment, there will be a $15 late fee applied to your total bill if Compassionate Groom and Spa is able to still accommodate you and your pet. If you are more than 10 minutes late to your appointment and your groomer cannot accommodate your pet, you will be considered a no show and charged the appropriate fee. If you were a "Fit in" appointment, the fit in fee and no show fee will be charged to the card on file. Exceptions of these fees are at the discretion of your groomer.
Card on File: I understand that my card on file is used solely for no-show or late cancellation protection. This is a service provided by our point of sale system and at no point does Compassionate Groom and Spa have full access to your credit/debit card information. A card on file is required to maintain an appointment at our salon. If we do not receive your card on file after 48 hours of communications, your appointment may be cancelled.You may use the card on file at checkout if you choose to, but you can provide payment via cash, check or another card. Sitting Fee: Your groomer will call you to establish a pick up time. The owner will have an additional 30 minutes from the scheduled pick up time to pick up their pets. If you do not answer your phone we will leave a message with the time when your pet will be done and the time we called. This is to maintain our facility's goal to have one on one grooming and keep stress levels down as much as possible, including for other pets in the building that are there for an appointment. Pets that are picked up after 30 minutes will be charged a sitting fee of $10 per 30 min after the pet has not been picked up. If you need to drop your pet off more than 30 minutes early to their appointment, the $10 sitting fee will apply for every 30 minutes prior. Waiving of these fees are at groomer discretion. If we are unable to get a hold of you to pick up your pet, or you have still not come for your pet by closing, a staff member will continue to attempt to get a hold of you for pickup up to 30 minutes AFTER closing. If we are unable to get a hold of you still or you are unable to arrive to pick up your pet, your pet will be fed a bland, dry diet, given fresh water, taken outside to the bathroom, and safely crated in our facility until we open the following business day. Compassionate Groom and Spa will ensure the utmost safety and care for your pet during their stay, but we are not responsible for anything that may happen due to, but not limited to, missing medications, medical emergency overnight, damage to self due to not being crate trained, etc. I understand that I am responsible for any damages my pet may cause during this time. The fee for this overnight stay starts at $60.
Matting/Impaction Fee: I understand that if my pet is very tangled, knotted, impacted, or matted there will be a matting or impaction fee that will be charged to me in addition to the original grooming cost. This fee depends on the severity of the matting, extra length of time, and the damage matting can cause to our equipment. I also understand that if my pet is matted there is a higher risk for nicks or razor burn due to needing to be able to get under the mat to remove these. Depending on the severity of the matting, I understand that while my groomer will put in all the effort they can to get through and comb/brush out these mats safely to provide a longer hair cut, my pet's safety and comfort is their first priority. This means if my pet will not tolerate the additional work and time needed to remove the matting or impaction, then my groomer will make the best decision for my pet, which may include taking the haircut much shorter than discussed or anticipated. Difficult Puppers/Kitty Fee: As a Fear Free Certified salon, our goal is to always provide your pet the safest, calmest grooming experience possible. Sometimes this may mean working with your pet through Behavior Modification Training, different grooming techniques, and even having your pet on medication from the Veterinarian. If my pet requires extra grooming time or more assistance throughout the grooming process due to behavioral issues, I understand there may be a Difficult Puppers or Feisty Feline Fee in addition to the grooming services. I understand if my pet is too fearful or aggressive to begin the groom, I may or may not be charged for the grooming package, but I may be charged a Difficult Puppers or Feisty Feline fee. If the groom was started but not completed, I understand I will be charged for the services and a Difficult Puppers or Feisty Feline fee. I understand if i knew my pet was aggressive from other groomers and did not medicate my pet appropriately before the appointment i will be charged for the appointment in full.
I agree that I have read and understood the rules and conditions for Compassionate Groom and Spa. I agree to release, hold harmless, acquit, and forever discharge Compassionate Groom and Spa or any other person I have with me during grooming, from all manner of actions that include suits, damages, judgements, executions, claims and demands whatsoever, in law or in equity. I understand that payment is due at the time of service, including any additional fees that may have been added. If I am unable to make payment I can discuss payment arrangement with management. If I fail to uphold the payment agreement I understand that there will be a fee equal to 25% of the balance added onto the bill and a final attempt for payment will be made. Afterwards, this balance is sent to collections. I understand this waiver is binding and by signing I agree with the above information. This waiver is valid for a year from this date.
