As a part of these efforts, the Club requires each participant, parent or guardian of a participant under the age of 18, Coaches, and Trainers to abide by a concussion specific Code of Conduct as it pertains to the sport of Curling. I will make the minor participant aware that they can help prevent concussions by: - Wearing the proper equipment for Curling and wearing it correctly.
- Developing their skills and strength so that the minor can participate to the best of their ability.
- Respecting the rules of Curling.
- Remaining commitmed to fair play and respect for all (respecting other athletes, coaches, team trainers and officials).
I, and the minor under my care and control, will prioritize their health and safety by taking concussions seriously, and both myself and the minor understand that: - A concussion is a brain injury that can have both short and long-term effects.
- A blow to the head, face or neck, or a blow to the body that causes the brain to move around inside the skull may cause a concussion.
- An individual doesn't need to lose consciousness to have had a concussion.
- Both myself and my minor must have a commitment to concussion recognition and reporting, including self-reporting of a possible concussion and reporting to a designated person when and individual suspects that another individual may have sustained a concussion. (Meaning: If my minor thinks they might have a concussion they should stop participating in further training, practice or competition immediately, or tell an adult if they think another athlete has a concussion).
- Continuing to participate in further training, practice or competition with a possible concussion increases one's risk of more severe, longer lasting symptoms, and increases their risk of other injuries.
I will ensure that the minor under my care and control knows not to and does not hide concussion symptoms. I will ensure that the minor knows to speak up for both themselves and others. - The minor will not hide their symptoms and will be instructed to tell a coach, official, team trainer, parent or another adult they trust if they experience any symptoms of concussion.
- I will ensure that the minor is aware that, if someone else tells them about concussion symptoms, or if they see signs that another participant might have a concussion, they will tell a coach, official, team trainer, parent or another adult they trust so they can help.
- The minor understands that if they have a suspected concussion, they will be removed from Curling and that they will not be able to return to training, practice or competition until the minor undergoes a medical assessment by a medical doctor or nurse practitioner and has been medically cleared to return to training, practice or competition.
- The minor has a commitment to sharing any pertinent information regarding incidents of removal from Curling with the minor’s school and any other sport organization with which the minor has registered (Meaning: If the minor is diagnosed with a concussion, they understand that letting all of their other coaches and teachers know about their injury will help them support the minor while they recover.)
I will ensure that the minor takes the time they need to recover, because it is important for their health. - Both myself and the minor understand and are commitmed to supporting the return-to-sport process (the minor will have to follow the Club’s Return-to-Sport Protocol).
- Both myself and the minor understand that the minor will have to be medically cleared by a medical doctor or nurse practitioner before returning to training, practice or competition.
- Both myself and the minor will respect the coaches, team trainers, parents, health-care professionals, medical doctors and nurse practitioners, regarding the minor's health and safety.
I will ensure that the minor is aware that they can help prevent concussions, through their: - Commitment to zero-tolerance for prohibited play that is considered high risk for causing concussions.
- Acknowledgement of mandatory expulsion from competition for violating zero-tolerance for prohibited play that is considered high risk for causing concussions (Meaning: the minor will be disqualified/expelled from play if they violate the zero-tolerance policy).
- Acknowledgement of the escalating consequences for those who repeatedly violate the Concussion Code of Conduct.
By completing this form, I acknowledge that both myself and my minor have fully reviewed and commit to this Concussion Code of Conduct. . |
