LIABILITY RELEASE:

The undersigned, herein referred to as "Child, Teacher, Studio Owner, Parent or Legal Guardian", understands that participation in Hollywood Connection, Inc. also doing business as Hollywood Connection Dance Convention and Competition, is completely voluntary and involves exposure to inherent risks that cannot be eliminated.  The undersigned fully understands that the risks and dangers may include but are not limited to illness, bodily injury, blisters, sprains, bruises, fractures, concussions and broken bones.  Although it is highly unlikely, the undersigned is aware that participation in any dance related activity including but not limited to classes, workshops, competitive dance and performances at any Hollywood Connection event involves exposure to inherit risks including serious injury, paralysis or death.  The undersigned acknowledges and agrees that Hollywood Connection, Inc. has not assumed any duty to minimize the risk of injury or the severity of any injury that may be suffered while participating in Hollywood Connection Dance Convention and Competition. The undersigned hereby expressly assumes all risks associated with participation in Hollywood Connection Dance Convention and Competition including classes, workshops, performances, competitions and any other activities while in attending.  The undersigned will be responsible for any and all medical and related bills and claims that may be incurred for any personal property loss, illness or injury sustained during Hollywood Connection Dance Convention and Competition.  The undersigned also understands that they may decline participation in classes, workshops, performances, competitions, and any other activities while in attendance.  

COVID 19:
The undersigned acknowledges there are risks involved with participating in person due to the contagious nature of Covid-19 and participation is voluntary and of their own free will. The undersigned assumes all risk by attending the event and understands that exposure to COVID-19 could result in serious illness, injury, paralysis or death.  The undersigned acknowledges the risk of being exposed to or infected by COVID-19 from the actions or negligence of others, including but not limited to event participants, employees and essential workers. The undersigned will be responsible for any and all medical and related bills and claims that may be incurred for any injury, illness or infection that occurs before, during or after the Hollywood Connection event.

COMMUNITY SAFETY AND DUTY TO SELF MONITOR:

The undersigned acknowledges temperatures may be taken at the event and if participant has a fever that is equal to or exceeds recommended guidelines, Hollywood Connection Dance Convention and Competition will decline the undersigned's participation in the event. The undersigned agrees not to attend the event if they show respiratory symptoms, have a fever of 100.4 or greater or live in a household with someone exhibiting fever or respiratory symptoms.  The undersigned agrees not to attend the event if they have tested positive or anyone in their family has tested positive for COVID-19 prior to the event. The undersigned agrees to follow current CDC guidance for isolation and exposure if they have tested positive or are awaiting test results. The undersigned will be responsible for physically distancing from others, maintaining good hygiene practices by washing hands frequently, using hand sanitizer, and wearing appropriate face coverings in public and shared spaces. The undersigned hereby agrees to release from liability and to defend, indemnify and hold harmless Hollywood Connection, Inc., and their representatives, owners, employees and agents for any illness, infection, damage or injury, paralysis, death, including personal property loss, arising out of the undersigned's participation in Hollywood Connection, Inc. regardless of the cause, including negligence.  

VIRTUAL SAFETY:
The undersigned acknowledges participation may occur in a home or studio setting. The undersigned agrees to dance on a safe surface and in a space adequate for movement, free of furniture and clutter. Undersigned agrees to wear proper dance shoes at all times.  The undersigned acknowledges they will participate with an adult present in the home or studio at all times.  The undersigned also acknowledges if they are participating in a studio setting, they will adhere to all studio protocols as it pertains to Covid-19 including but not limited to mask wearing (if applicable) and physical distancing guidelines.

The undersigned acknowledges the LIABILITY RELEASE is legally binding and signs it willingly.

MEDICAL RELEASE:
The undersigned hereby gives consent to Hollywood Connection, Inc. and the host hotel or facility, authorization to obtain necessary medical treatment on the behalf of the undersigned including but not limited to transportation and emergency medical services.  The understanding agrees that neither Hollywood Connection Inc., nor the host facility, venue or organization of any Hollywood Connection event, has any obligation to provide any such medical attention. The undersigned is responsible for any and all medical and related bills and claims that may be incurred during medical treatment.  

SUPERVISION: 
A chaperone (age 21 or older) is required to attend with participants, including minors.  This chaperone will be responsible for the participant at all times.  Hollywood Connection, Inc. is not responsible for participants' supervision.
