SHRINK WRAP AGREEMENT

This is a contract & work order for the installation of a shrink wrap cover by Elevated WaterSports LLC over a vessel as described below with responsibility for acceptance of contract terms and payment to be assumed by the party completing this form (Customer), whether vessel owner or representative party thereof.

Installation Details

Our shrink wrap installation price does not include the construction of a lumber framework to prevent snow build up. We o er frames at an additional cost of $50. The price includes all necessary accessories (vents, tape, strapping) and preparation required. Our shrink wrap cover consists of 7 mil, white or blue shrink wrap film, and after heat shrinking the cover we will inspect for and tape any holes created

during the heat shrinking process and will install an adequate number of vents to allow for passive airflow to reduce the potential for mold/ mildew to develop under the cover during the storage period. Elevated WaterSports LLC cannot be responsible for mildew growth during storage of boat. You have the option of installing your own framework prior to shrink wrap installation, however, any modification we need to make in order to have your framework meet our approval for shrink wrap installation will incur an additional charge. We do not guarantee a shrink

wrap cover if we have not installed the framework.

Please note that shrink wrap removal & recycling is NOT included in this service but is available at an additional charge for customers who have had us install the shrink wrap.

Payment Details

You have a couple of di erent options to select from to pay for your shrink wrap installation, but in all cases we must have payment information prior to scheduling your service date.

All invoicing and credit card payment will be done via Square. If you wish to pay by Cash or Check: All payments must be received in advance of the service date or you may present a check to us at the time of service. No exceptions.

Elevated WaterSports LLC and/or their agents are not a named insured and shall not be liable for loss due to fire or other causes not occasioned by a failure by it or one of its agents or employees to exercise due care. Elevated WaterSports LLC assumes no responsibility for items of personal property left in the boat during storage. Elevated WaterSports LLC shall have a lien on the boat for all maintenance, repair and storage fees and costs. Elevated WaterSports LLC may, at its option, retain possession of the boat until all fees and costs are paid and Owner shall be liable for all reasonable attorneys fees and costs should it be required to retain an attorney to enforce the terms of the agreement.
