Southern Idaho Brazilian Jiu Jitsu

The Member (the individual identified above or authorized by a parent, if a minor child) hereby submits this Membership Agreement to Southern Idaho Brazilian Jiu Jitsu, LLC d/b/a SIBJJ (“SIBJJ”).

WAIVER AND RELEASE OF LIABILITY AGREEMENT

The Member (if over the age of 18), in consideration of being permitted to participate in any way with classes offered by SIBJJ, acknowledge and agree to release, indemnify, waive and discharge, to the greatest extent permitted by law, SIBJJ from or for all claims, demands, and causes of actions or any other liabilities which may arise or be caused in whole or in party by the negligence of SIBJJ, its owners, employees, agents or assigns and the action or lack thereof of the same.  The Member agrees that the Member knows and understands the risks involved in training Brazilian Jiu Jitsu (or any other martial arts) may include serious or permanent injury or death and does hereby assume these risks and accepts responsibility for any damages or injuries by engaging in the contact sport of Brazilian Jiu Jitsu.

RULES AND POLICIES

The Member agrees to follow the rules and policies of SIBJJ, which may be communicated in writing or orally through SIBJJ or its designated agents or teachers.  Furthermore, the Member agrees to adhere to the instruction of any teacher during any class offered by SIBJJ and will communicate with the same if any instruction, rule or policy is not understood.

PARENT / LEGAL GUARDIAN INDEMNIFICATION The Parent identified above states that he/she is the parent or legal guardian of the above-described Member, a minor. The Parent agrees to indemnify and hold harmless SIBJJ for any expenses incurred, claims made, or liabilities assessed against them as a result of any injury, death, or insufficiency of legal capacity. The Parent consents to the Member participating with SIBJJ’s Brazilian Jiu Jitsu practices, lessons, clinics and events and understands the risks involved in training Brazilian Jiu Jitsu as stated herein. Additionally, the Parent agrees to follow the rules and policies of SIBJJ, which may be communicated in writing or orally through SIBJJ or its designated agents or teachers.
