Mandatory Waiver and Release of Liability Form 

I agree and acknowledge as follows:

  1.  MISH MASH Adventures places you in rooms where you and others in the room (your team) will complete challenges. Rooms involve physical challenges and may contain tripping and/or falling hazards. These physical challenges may include, throwing objects, kicking objects, climbing, ducking and jumping. Since you are completing the challenges with others in the same room, you could also become injured by physical contact with other players on your team.  Please note that you may encounter lasers and strobe style lights in some rooms. I fully understand and acknolwedge that (a) risks and and dangers exist in my participation with MISH MASH Adventures; (b) my partcipation in the challenges may be physical and/or emotionally demanding and may result in bodily injury; (c) these risks and dangers may be caused by negligence of the owners, employees, officers or agents of MISH MASH Adventures, my negligence, the negligence of other participants, the negligence of others, the forces or nature, or other causes. These risks and dangers may arise from forseeable or unforseeable causes and by my participation in these activities. I hereby assume all risks and dangers and all responsibility for any losses and/or damages, whether caused in whole by the negligence or other conduct of the owners, agents, officers, employees of MISH MASH Adventures or by any other person. 
  2.  I, on behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify MISH MASH Adventures, its affilates and related companies and their respective directors, managers, owners, officers, successors, assigns, agents, representatives and employees (collectively, "Released Parties") from any and all claims, actions or losses for bodily injury, emotional distress, pain and suffering, property damages, wrongful death, or otherwise which may arise out of my participation with MISH MASH Adventures. I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have now or in future for the negligence or other conduct by the owners, agents, officers or employees of the Released Parties. 
  3.  I understand that inappropriate behavior that interferes with the delivery of the guest experience, including but not limited to, being under the influencer of alcohol and/or drugs, may result in me being turned away and my game stated as a now show.
  4.  The Released Parties are not responsible or liable for my personal belongings while at MISH MASH Adventures.
  5.  I give MISH MASH Adventures the right to use any photographs or video recordings created while I parcitipate in MISH MASH Adventures for publicity and/or advertising purposes. 
  6.  I acknowledge that the contact information set forth below is true and accurate.
  7.  I hereby grant MISH MASH Adventures to store my contact information for operational purposes, unless I grant the permission as set forth below.

By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature.
