By signing this agreement, you waive certain legal rights, including the right to sue.

1. Acknowledgment of Risk

I, the undersigned participant (or legal guardian of a minor participant), acknowledge that participation in [Insert Name of Sport or Activity] at [Insert Facility Name or Event] involves inherent risks, including but not limited to serious injury, permanent disability, illness, or death. These risks may result from my actions, the actions or inactions of others, the conditions of the premises or equipment, weather, or the nature of the activity itself.

I freely and voluntarily choose to participate and expressly assume all risks, known or unknown, whether caused by negligence or otherwise.

2. Waiver and Release

To the fullest extent permitted by New Jersey law, I hereby waive, release, and discharge [Insert Business/Organization Name], its officers, employees, volunteers, affiliates, contractors, landlords, and agents (“Released Parties”) from any and all liability, claims, demands, causes of action, or expenses (including attorneys' fees) arising from or related to any injury, illness, loss, or damage sustained by me (or my minor child) while participating in the activity, whether caused by the negligence of the Released Parties or otherwise.

3. Assumption of Responsibility

I understand and agree that I enter and participate at my own risk, and that the Released Parties make no guarantee of safety. I am solely responsible for assessing my physical condition and ensuring that I am fit to participate. I further agree to follow all rules, instructions, and warnings provided by the organizers or facility staff.

4. Indemnification

I agree to indemnify and hold harmless the Released Parties from any and all claims, damages, or losses brought by me, my family, estate, heirs, or assigns, or any third party, arising from my participation in the activity.

5. Severability and Governing Law

If any provision of this agreement is found to be unenforceable, the remaining terms shall remain in full force and effect. This agreement shall be governed by the laws of the State of New Jersey.

6. Parental Consent (if under 18)
