12-MONTH EFT MEMBERSHIP CONTRACT


SUBJECT OF THE CONTRACT

Unlimited access membership paid by electronic funds transfers(EFT) on the 1st of each month for 12 consecutive months, valid at all Bloc Shop Climbing Center locations.

Preferential monthly rate (before taxes): $64.00

PAYMENT TERMS

Upon enrollment, the client pays a prorated amount for the remaining days until the 1st of the following month (i.e., the current month). The next 12 payments will be automatically charged on the 1st of each month to the credit card provided.

At the end of the 12 months, the automatic withdrawals will stop. The membership will not be renewed automatically.

RIGHT OF CANCELLATION

The consumer may cancel this contract at any time by completing the form available under the “Manage My Membership” tab on the website.

The contract will be terminated on the next 1st of the month.

A cancellation fee of $75 will apply and will be charged to your credit card.

 By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
