Bowling at Edgewood Bowling Alley

By signing this document I am agreeing to the following:

To allow my child to ride in the van and participate in bowling at Edgewood Bowling Alley in Neosho on March 27th at approximately 6:00pm-9:00pm.

I also understand that my child is expected to behave appropriately throughout this entire trip and any action deemed inappropriate could result in my student being sent home early. If my child must return home early due to disciplinary issues, it will be at my own financial expense, and not Seneca Christian Church. I consent to the use of any video, images, photographs, audio recordings, or other visual or audio reproduction that may be taken of the subject of this release during the activity/event to be used, distributed, or shown as Seneca Christian Church sees fit. I understand all reasonable safety precautions will be taken at all times by Seneca Christian Church and its agents during the events and activities. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree not to hold Seneca Christian Church, its leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred by the subject of this form.
