1. I do hereby that me/my child is not having any signs or symptoms of respiration infection, including difficulty breathing; Fever; Chills; Muscle pain; Sore throat; also he didn’t travel outside USA on the past 14 days and din’t have shortness of breath or difficulty breathing; Fever; Chills; Muscle pain; Sore throat; also he didn’t travel outside USA on the past 14 days and din’t have shortness of breath or had any contact with COVID-19 patient.

2. Recognizing the possibility of injury associated with soccer and in consideration for High Soccer Arena programs, I hereby fully and forever release and discharge High Soccer Arena, their employees or any of its representatives, coaches, assistants, agents or assigns, including the owners of fields and facilities utilized for programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs.

3.. In consideration for accepting me/ or my child into the High Soccer Arena, I do hereby agree that I am and shall be responsible for all costs associated with any injury or loss that may be sustained by my child as a result of his or her participation in any activities at the field. I further understand that in the event of a medical emergency, High Soccer Arena will call Emergency Medical Services to render assistance and that I will be financially responsible for any expenses involved

4. Waiver of Liability: In consideration of being permitted to play soccer, on behalf of myself, my family, my heirs, and my assigns, I hereby release High Soccer Arena, its agents, and its employees from liability for injury, loss, sickness or death to the above-mentioned participant while using any facility or equipment or in any way associated with participating in the activity of soccer now or in the future, resulting from the ordinary negligence of High Soccer Arena, its agents and employees.

Attention: Our Soccer goals are not bolted to the ground, is not allowed to climb on the goals, is extremely prohibited playing soccer at this area, the lights may breaks and it's very dangerous.

Our Fields are made with acrylics on the face parts, is not allowed to hit it using your feet or hands with the intention of breaking it.
